Community Resource Specialist
The purpose of this position is to facilitate independent living for older and/or disabled adults and their families by helping to sort through all options, both publicly and privately funded. The Community Resource Specialist (CRS) is an access point provider to the “no wrong door” system for person centered planning including needs assessments, referrals, preliminary care planning, and short-term tracking based on consumer choices for persons in need of Long Term Supports and Services (LTSS). The CRS serves older adults, all persons with disabilities, family caregivers and veterans. This work is in collaboration with LTSS providers. Counseling provides accurate and unbiased information, guidance, advocacy and support. Counselors give a voice to the needs of the individuals while helping to provide dignity as people age in place.
Duties
- Completing in-home assessments, generating reports regarding assessments, and facilitating a plan with individuals and their support system within the scope of the funding source.
- Responds to inquiries from individuals, businesses and agencies requesting information on Community Resources, in a professional, non-judgmental manner, recognizing the consumer’s right to make his or her own choices.
- Assists consumers via telephone, email, face-to-face visits, or home visits.
- Completes semi-structured assessment of consumer’s concerns, problems, needs, eliciting information (including exploring the consumer’s own resources) necessary for accurate referrals and options counseling.
- Performs person centered comprehensive needs assessments, including mental, physical, functional, cultural, financial, environmental, and life goals to determine appropriate referrals
Skills required:
- Effectively communicate with consumers and families including those in crisis or with special needs, professional staff and the general public with tact and courtesy.
- Excellent organizational and communication skills (verbal & written).
- Ability to prioritize multiple needs and determine appropriate information, referrals and resources.
- Capable to work both independently and collaboratively
- Ability to maintain confidentiality of client information.

BS in Social Services or Healthcare related field required.Similar education or experience considered.
