Community Outreach Coordinator - Part Time

Job Summary:

The Department of Obstetrics and Gynecology at SUNY Downstate Health Sciences University is seeking a part-time Community Outreach Coordinator. The successful candidate will:

• Manage programmatic partners and establishes referral pathways.
• Develop a portfolio of 15 to 25 organizations and cultivates key relationships at all levels of staff as well as with clients.
• Achieve visit targets set by management.
• Coordinate outreach events and programs.
• Build relationships with internal programs and services and act as a liaison between external referral sources and internal programs.
• Track outreach activities in agency Salesforce CRM or tracking tool.
• Create and implement new marketing strategies and plans to achieve targets.
• Participate in Community Engagement team meetings and participate in team huddles.
• Complete required documentation.
• Assist client with linkage to intake and assessment services.
• Organize health home intake assessment days at community partner sites.
• Participate in presentations to advance relationships with key target audiences.
• Locate and participate in Health Fairs, Conferences, and other events to increase awareness and visibility. Evening and weekend work may be required.
• Perform other duties as assigned by Supervisor.

Required Qualifications:

• High School Diploma or equivalent.
• 2+ years of applicable experience and/or equivalent experience.
• Proficient in Microsoft Office.
• Excellent communicator with a willingness to learn and collaborate with others.
• Strong work ethic.

Preferred Qualifications:

• Experience with virtual outreach and graphic design a plus.
