As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW’s work in a team - based structure and spend the majority of their time in the community engaging directly with members. In this role, you will serve as a Justice System and Court Liaison and will be the primary point of contact to respond to inquiries from the Health Care Authority (HCA), Child, Youth Families Department (CYFD), Members/families, Caregivers, and Providers, and will work collaboratively to address concerns related to the delivery of and access to services for Children in State Custody (CISC) Members and Child Members in out-of-home placements.
This position is full - time (40 hours / week) Monday - Friday. Employees are required to have flexibility to work any of our 8 - hour shift schedules during our normal business hours of 8am to 5pm. It may be necessary, given the business need, to work occasional overtime. This position is a field - based position with a home - based office. You will work from home when not in the field.

Primary Responsibilities:
- Create a positive experience and relationship with the member
- Practice cultural sensitivity and cultural competence in daily care
- Learn and listen to member needs and barriers to help promote self advocating
- Work with care coordinator to develop a care plan and set goals for member care
- Collaborating with clinical team of social aspects that might impact treatment plan
- Proactively engage the member to manage their own health and healthcare
- Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
- As needed, help the member engage with mental health and substance use treatment
- Support the member to ensure pick-up of their prescriptions
- Provide member education and health literacy on community resources and benefits to encourage self sufficiency
- Conduct post discharge activities from hospital facility and support connection to social services
- Support member to engage in work or volunteer activities, if desired, and develop solider social supports through deeper connections with friends, family, and their community
- Partner with care team (community, providers, internal staff)
- Knowledge and continued learning of community cultures and values
- Conduct Health Risk Assessments (HRA)

Required Qualifications:
- 1+ years of experience working with New Mexico community resources
- Lived or work experience with the Justice System
- Knowledge of culture and values of the community
- Familiarity with the resources available in the community
- Ability to navigate a PC to open applications, send emails, and conduct data entry
- Must have a designated workspace inside the home with access to high - speed internet availability
- Ability to travel locally up to 25% of the time
- Must reside in the State of New Mexico
Preferred Qualifications:
- CHR/CHW Accreditation
- 2+ years of field - based experience
- Care management experience
- Experience working in Managed Care
- Experience with Justice System or Court System
- Knowledge and / or experience with behavioral health or substance use disorders
- Knowledge and training on Social Determinants of Health (SDOH)
- Knowledge of Medicaid and / or Medicare population
- Must have lived in the state of New Mexico for 2 years
