As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.  CHW's work in a team - based structure and spend most of their time in the community engaging directly with members.  In this role, you will serve as a liaison for Children's Services and School-Based Services Liaison. For School-Based Services, you will coordinate with the New Mexico Department of Health, Office of School and Adolescent Health; schools; school districts; and Providers to expand services delivered in School-Based Health Centers.  As a liaison for Children's Services, you will serve as a primary point of contact to respond to inquiries from HCA, CYFD, Members/families, Caregivers, and Providers. 


 
This position is full - time (40 hours / week) Monday - Friday. Employees are required to have flexibility to work any of our 8 - hour shift schedules during our normal business hours of 8am to 5pm.  It may be necessary, given the business need, to work occasional overtime. This position is a field - based position with a home - based office. You will work from home when not in the field.

Primary Responsibilities: 

  • Work collaboratively to address concerns related to the delivery of and access to services for CISC Members and child Members in out-of-home placements
  • Partner with care team and other stakeholders (school leaders, The Alliance for school-based health centers, community, providers, internal staff) to increase us of school-based health centers by youth
  • Create a positive experience and relationship with the member 
  • Practice cultural sensitivity and cultural competence in daily care
  • Learn and listen to member needs and barriers to help promote self-advocating
  • Work with care coordinator to develop a care plan and set goals for member care 
  • Collaborating with clinical team of social aspects that might impact treatment plan. Proactively engage the member to manage their own health and healthcare
  • Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
  • As needed, help the member engage with mental health and substance use treatment
  • Provide member education and health literacy on community resources and benefits to encourage self sufficiency
  • Conduct post discharge activities from hospital facility and support connection to social services
  • Knowledge and continued learning of community cultures and values
  • Conduct Health Risk Assessments (HRA)

Years of post-high school education can be substituted/is equivalent to years of experience

Required Qualifications: 

  • 2 years of experience working with youth and children
  • Knowledge of culture and values of the community
  • Familiarity with the resources available in the community
  • Ability to navigate a PC to open applications, send emails, and conduct data entry
  • Ability to travel locally up to 25% of the time
  • Must have a designated workspace inside the home with access to high - speed internet availability 
  • Reside in the State of New Mexico

Preferred Qualifications:         

  • CHW Accreditation
  • 2 years of field - based experience working with youth and children
  • Experience working in Managed Care
  • Experience with school-based health centers
  • Knowledge and / or experience with behavioral health or substance use disorders
  • Knowledge of Medicaid and/or Medicare population
  • Have lived in the state of New Mexico for 2 years
