The Asthma/COPD Community Health Worker works as a member of the primary care team through the Asthma/COPD Program to improve the health and quality of life of Boston-area and Winthrop residents living with asthma and COPD. Through home visiting and within the clinic, the CHW delivers asthma/COPD education, reinforces medication adherence and environmental/home interventions and delivers smoking cessation support to improve asthma/COPD symptom control and long-term management.
The CHW also performs a variety of activities that support the delivery of Asthma/COPD services to patients of the Family Medicine Asthma/COPD Program. The CHW supports efficient workflows by assisting in scheduling medical appointments, facilitating community resource access and supporting efforts for community outreach. The CHW works within the primary care department at Winthrop Family Medicine on a team consisting of the Asthma/COPD Team Leads and alongside other CHWs. The CHW will report to the EBNHC Asthma/COPD Program Manager.
Full-time position, Monday-Friday, 8:00 AM-5:00 PM.
Bilingual (English/Spanish) and access to reliable transportation/car required.
Responsibilities:
- Completes Boston Public Health Commission CHW Certification Program (COEC)
- Completes Department of Public Health Asthma Home Visit training
- Completes Smoking Cessation training.
- The Asthma/COPD CHW schedules and conducts home visits and assessments to evaluate for asthma triggers, provides asthma/COPD education per home visit protocols, provides smoking cessation support, and care coordination services for patients of EBNHC identified by the care team.
- Conducts follow-up visits to reinforce education and assess maintenance of intervention
- Conducts Smoking cessation group workshops in the community
- Documents all visits, activities with family, and supplies used and enters data into the electronic medical record
- Connects families to community agencies, social service organizations, insurance providers and other resources according to identified needs.
- Responsible for appropriate use of intervention supplies, maintaining organization of storage space and documentation of use of supplies.
- Attends all regularly scheduled staff meetings, including Asthma/COPD team meetings and department meetings at site.
- Maintains scheduled visits on electronic medical record template.
- Assist patient with forms and coordination of care activities
- Follows established departmental policies and protocols.
- Follows all HIPPA and confidentiality regulations regarding the protection of patients
- Assists with Community Health events when appropriate.
Experience:
- Understanding of language, culture, and socioeconomic circumstances and desire to work with diverse population is a must.
