Seven (7) full-time (40 hours/week) position as Community Health Workers for our new Enhanced Care Management program. The Community Health Worker (CHW) is a paraprofessional or peer advocate who provides administrative support to a lead care manager. CHW engages eligible Enhanced Care Management (ECM) members and provides and coordinates services as a part of a multidisciplinary team. 

Qualifications:

  1. High School diploma preferred.
  2. Lived experience that aligns with and provides a connection with population being served.
  3. Experience of 2,000 hours working as a CHW in paid or volunteer positions within the previous three years or a CHW certificate of completion within 18 months from start of employment, including but not limited to any certificate issued by the State of California or a State designee.
  4. Intermediate computer knowledge and experience working with word processing and data entry programs.
  5. Knowledge of community resources.
  6. Bilingual in Spanish preferred.
  7. A valid California driver’s license with less than two minor moving violations is  
    required of staff who have driving duties to meet the requirements of their
    position and be approved by the agency’s insurance company.

Responsibilities:

  1. Utilize prevention strategies to promote stigma and discrimination reduction.
  2. Provide access, advocacy, resources, and linkage to clients and their families.
  3. Provide outreach activities and services with a focus on physical, mental health and well-being services and support.
  4. Interact productively within a multi-disciplinary team and other community organizations.
  5. Maintain documentation, record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  6. Attend community events.
  7. Responsible for preventing and controlling infection.
  8. Responsible for maintaining a culture of quality and safety.
  9. Effectively use de-escalation skills and techniques for crisis prevention.
  10. Properly administer the C-SSRS to help prevent suicide.
  11. Helps engage and enroll eligible members in ECM.
  12. Call members to facilitate visit with lead care manager.
  13. Utilize motivational interviewing, trauma-informed care and harm-reduction strategies.
  14. Advocate on behalf of members with health care professionals.
  15. Assist with linkage to social support.
  16. Connect members to other social services and support including transportation.
  17. Distribute health promotion materials.
  18. Provide health promotion and self-management training.
  19. Accompany members to office visits, as needed.
  20. Monitor treatment adherence.
