Form of workMaxim Healthcare Staffing is looking for qualified Community Health Worker (CHW) to be responsible for providing reentry services to program participants. The CHW will be assisting in jail and in community population.
- Working inside Correctional facility
- No weekend or evenings (M - F)
- Day shift 7am-3:30pm (8 hours)
- Full Time 40 hour work week
- Entry level with 6 months of experience

Responsibilities
- Conduct follow ups, and IRP's within the correctional facility.
- Candidate will plan and schedule meetings and appointments.
- Organize and maintain paper and electronic files.
- Conduct research and provide information via the telephone, postal mail, and e-mail.
- Other Administrative Assistants duties as assigned.
Qualifications
- High school diploma or GED required
- Ability to work independently with little direct supervision.
- Ability to establish and maintain positive professional business relationships with the branch level leadership and office staff.
- Strong organizational skills and attention to detail.
- Excellent oral and written communication skills.
- Experience with Microsoft office required.
