Job Description
Community Outreach Specialist provides activities and programs that improve the health and overall quality of life of community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CareSouth Carolina into the community; and engaging residents in improved individual health. Duties include door-to-door outreach, community/neighborhood canvassing; successful medical home and social service referral; increasing public education and awareness; the accurate reporting of information and data; positive public relations; and the integration of community services into CSC's current delivery of care.
Education and Experience
- High school diploma or GED Equivalent required
- One to three years of related experience required
- Valid driver's license required
- Must have excellent verbal and written communication skills
- Microsoft Office experience required
- Must have the ability to work effectively independently and as part of a team
Location
Cheraw, SC
Hours
Office hours are 8:30 - 5:00. Due to the nature of the position some weekend and evening work is required.
