Position Summary:
The Community Health Worker provides activities and programs that improve the health and overall quality of life of patients and the community we serve. The focus of the CHW is to help the patient population by decreasing barriers that may prevent them from accessing health and medical care. The goal is to engage patients in improving their health through individual health education, follow up, and prevention. The CHW will play a key role in increasing public and patient health education and awareness, accurately reporting information and data, addressing social barriers to health and the integration of community services into Rural Health Services’ current delivery of care. This position requires some knowledge and comfort level of working with patients diagnosed with diabetes and other chronic diseases in a primary care setting.
Essential Functions:
- Frequent travel from site to site for patient enrollment/education.
- Planning Covid Vaccine events: Completion of consent forms, patient check in/out and uploading consent forms into EHR.
- Schedule appointments for all referrals received for CDE.
- Monitor all RPM dashboards and contact patient for alerts and missing data. Provides documentation and follows patient’s adverse events through resolution.
- Conducts weekly follow up calls to CHW patients
- Coaches patients in effective management of their chronic health conditions and self-care habits, such as preventing and managing uncontrolled diabetes.
- Assists patients with self-management of chronic health conditions and medication adherence.
- Facilitates communication and coordinate services between providers.
- Collects, reports, and enter data for purposes of evaluating CHW activities and completing grant reporting.
- Communicates with patients after a hospital discharge to follow up on treatment plans.
- Helps patients connect with transportation resources.
- Other duties as assigned.
- High school diploma required.
- Certification in Community Health Care
- Previous experience working in a health care setting required.
Preferred Qualifications
- Successful completion of a Certified Community Health Worker program
- Successful completion of a Certified Nursing Assistant Program or Certified Medical Assistant program.
- No less than 2 years of previous working experience as a Community Health Worker.
- Bilingual preferred.
