We are currently hiring a Community Health Worker, to work in our Care Coordination Office. As the Community Health Worker, you would be responsible for:
- Delivery of National Diabetes Prevention Program through outreach and engagement of the target population
- Assessing and assisting individuals in meeting goals
- Facilitating groups while utilization the required session materials
Community Health Worker Minimum Salary: $23.07
Community Health Worker Maximum Salary: $23.07
Community Health Worker Schedule: Mon-Fri 8am-4pm
Qualifications:
- High School Diploma or GED
- 1-3 years of related experience (willing to consider significant life experience for professional experience)
