POSITION: Care Manager I: Community Health Worker

RESPONSIBLE TO: Manager, Care Management

JOB SUMMARY:

Overall duties will be a balance between supporting primary care practices in assuring delivery of high quality care and supporting their patients in the self-management of identified health opportunities by assessing and addressing social determinants of health.

Principal Duties and Responsibilities:

Working with patients and patient care teams to promote the health and well-being of patients served. This includes, but is not limited to:

• Conduct post-emergency department visit follow-up phone calls
• Conduct on-site in-person emergency department visits with patients
• Assess, intervene and reduce avoidable emergency department visits and hospitalizations, as well as identify treatment opportunities
• Provide patient education as it relates to lifestyle changes, obtaining care at most appropriate locations and/or assessing and addressing social determinants of health
• Develop and implement individualized patient health care improvement plans
• Communicate and coordinate patient health care plan with patient care team
• Motivate patients to be active, engaged participants in their health and well-being
• Support patients with health lifestyle change efforts through scheduled telephonic visits
• Assist patients with the creation of and compliance with self-management plans
• Identify community resource needs and create referrals as indicated
• Update community resources annually and as needed
• Expand knowledge and understanding of community resources, services and programs provided
• Assess patient health opportunities and refer those with complex needs to Care Manager II
• Work daily reports and Monitor departmental faxes and refer to Care Manager II as appropriate
• Other duties as assigned

Required Qualifications:

• College/vocational training in health care related field, including, but not limited to Certified Medical Assistant (CMA)/Registered Medical Assistant (RMA)
• At least three years of experience in relevant field
• Interpersonal communication skills

Preferred Qualifications:

• Exceptional verbal and written communication skills including medical terminology
• Relevant experience and expertise in patient education, health education, and/or care navigation
• Ability to develop and maintain positive, supportive relationships with all members of patient care teams and the patients we serve
• Attention to detail
• Ability to successfully utilize Electronic Health Record, Practice Management System, Microsoft Office, the World Wide Web, and electronic mail
• Successful completion of Community Health Worker formal certification training program within 6 months of hire

Pay: $17.00 - $18.00 per hour

Expected hours: 40 per week
