About the Department
The Chicago Center for HIV Elimination (CCHE), is situated within the Biological Sciences Division at the University of Chicago Medicine. CCHE aims to eliminate all new HIV transmission events by 2041.
The University of Chicago's position on the South Side makes it uniquely situated to test, treat, and prevent new HIV infections in a highly affected population. The South Side communities and majority Black suburbs connected to them to the South represents the largest contiguous Black population in the United States. By using our integrated HIV transmission elimination approaches, we are beginning to make new infections in these communities less frequent.
The South Side Health Home is funded by the Chicago Department of Public Health to provide HIV care and prevention services to residents of Chicago's South Side. The University of Chicago Ryan White adult HIV care clinic is the main site within the South Side Health Home. This role will be based at the University of Chicago Ryan White adult HIV care clinic.
Job Summary
The Community Health Worker will be responsible for recruiting participants for a research study to improve screening for and treatment for substance use disorder among people with HIV. They will conduct educational and motivational sessions with clients living with HIV to engage them in use of the electronic patient portal associated with the electronic medical record. The electronic patient portal empowers clients by giving them access to their personal health information and allows them to communicate with their medical providers. The candidate will be based at the Ryan White adult HIV care clinic at the University of Chicago, and will work with a variety of populations living with HIV, including individuals with substance use disorder, young adults as well as older adults, and Black sexual and gender minority populations residing in Chicago's South side. This candidate will actively pursue required knowledge and skills for professional development in regards to the aforementioned populations and topic area. They will stay informed about the electronic patient portal as well as resources available on the south side of Chicago that serve our population.

Responsibilities
- Recruit and consent participants for a research study to improve screening for and treatment of substance use disorder among people with HIV.
- Provide education to clients about the functions of the electronic patient portal and provide training on ways to optimize use of the electronic patient portal.
- The candidate will facilitate screening for substance use disorder among clients with HIV within the electronic patient portal.
- Identify clients not enrolled in the patient portal and reach out to them to provide assistance to enroll if desired.
- Assess needs of clients living with HIV and substance use disorder and provide resource counseling and navigation.
- Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
- Work collaboratively and effectively within a team.
- Establish positive, supportive relationships with clients.
- Coach clients in effective management of their chronic health conditions and self-care.
- Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
- Motivate clients to be active, engaged community member in their health.
- Effectively work with people (staff, community members, providers, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between community members and institutions.
- Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program.
- Collects and analyzes data used to promote health education programs that help individuals, families, and communities maximize and maintain healthy lifestyles. Assists in development of curriculum, policies, workshops, and promotional materials with guidance and direction.
- Supports individual and Community Health by contributing to the building and maintaining working relationships with those in public health care. Serves as an educational resource and patient advocate.
