POSITION SUMMARY:
Reporting to the Community Outreach Supervisor, the Community Health Worker is responsible for assisting in the execution of community events and outreach activities for Family Christian Health Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Coordinates registration at community testing events.
• Connects with and follows up with community members who attend FCHC-sponsored or FCHC-participating events to determine resources needed.
o Contacts community members to connect them with FCHC-provided services, such as primary and OB care, behavioral health, health education, medication access, dental, nutrition, case management, Specialist referrals, etc.
o Determines additional services needed that are a barrier to healthcare, such as housing, transportation, food insecurity, WIC, peer support, employment, childcare needed by community members and connecting them to those resources.
o Maintains a current resource list with current contact information on health-related topics and key community service areas of importance to our patient population.
o Shares resource list throughout FCHC departments.
• Supports organization and execution of community events on behalf of FCHC.
• Assists in stocking equipment and supplies needed to conduct events.
• Coordinates virtual meetings, including organizing and administering Zoom and Teams meeting programs.
• Supports Quality and Marketing, FCHC Events Committee and Clinical Teams in outreach to patients.
• Distributes patient- and community-focused communication campaigns on behalf of the organization and specific clinics in the assigned service area and other areas of interest.
• Provides outreach support for site-specific clinical and program initiatives.
• Coordinates staff and volunteers to staff patient and community outreach events and initiatives.
EDUCATION, TRAINING AND EXPERIENCE
• High School Diploma or equivalent required, some college preferred
• Completion of Community Health Worker Training Certification
• 1+ years of Community Health Outreach experience
• Medical Assistant/Certified Nursing Assistant experience preferred.
• Bilingual preferred (English/Spanish)
• Equivalent combination of experience/education.

• Proficient in EMR (Centricity/Athena a plus)
• Proficiency with Microsoft Office applications, particularly Word, Excel, Outlook, PPT, Teams, SharePoint and One Drive.
• Proficiency with virtual meeting and scheduling software.
• Ability to collaborate with team members and relate well to people from diverse ethnic and cultural backgrounds, as well as have a passion for working with at-risk, culturally diverse populations.
• Willingness to work flexible hours in order to meet the organization’s needs/demands.
