Job description

This recruitment will establish an employment list to fill current and any future vacancies within the Health and Human Services Agency. The anticipated life of the employment list is six months. Current vacancy is with the Health and Human Services Agency located in Visalia.

Typical Duties

Meets patient in clinic, facility or at home to help identify social determinants of health impacting patients' health and general well-being Collaborates with social worker and/or care manager to develop action plan; develops trusting relationship with patients by providing support and advocacy to help achieve health goals; assists patients in accessing health, mental, social-related services, and community resources, such as accompaniment to specialist appointments and assistance with enrollment forms as needed to also include transportation; facilitates communication between all parties (patients, families, colleagues, and community-based organizations) as needed; attends meetings as required Communicate, represent, and promote the peer and family/caregiver perspective within the mental health system; offer encouragement, mentoring, and advocacy, and perform problem-solving on a one-to-one basis to help consumers obtain the necessities of daily living; familiarizing client with approved methods of preventive, supportive, and rehabilitative health care; acting as a liaison/advocate for clients in obtaining medical or mental health care and social services; educate consumers, family members, and caregivers about the wellness and recovery process, including individuals living with co-occurring disorders; assist consumers with navigation of the Tulare County Integrated Service delivery system in order to ensure needs are met by the appropriate caregiver; maintain and ensure HIPAA, ethical standards, and professional boundaries with all staff and clients; contribute to an organizational culture that respects and celebrates the diversity of all consumers; may contribute to multi-disciplinary treatment teams; may assist with family liaison work, including reunification; provide documentation of consumer services and encounters following all guidelines and policies and procedures; perform a wide variety of, keyboarding, and specialized and/or general clerical duties related to assigned functional area and Branch.

Employment Standards

Education: Requires High School diploma or GED.

Experience:
One (1) year experience in a healthcare, community-based, or social work environment

Knowledge of:
Mental illnesses and the system of care through “lived experience” with a mental health, substance use disorder, or homelessness, of a family member or personal; good organizational skills, interpersonal skills, and communication skills; programs and treatment techniques for the support and care of persons with mental illness; concepts of mental illness and mental health: the physical, emotional, and social needs of persons with mental illness; community resources and techniques for building rapport with clients; cultural and socio-economic stigma influencing mental health, substance use disorders, and homelessness.

Skill/Ability to:
Relate and communicate effectively with peers, family members, and the community served; relate to clients through sharing of success stories; understand and follow verbal and written instructions; maintain confidentiality of consumers’ information; retain and recall information pertaining to specific procedures; lead and interact with clients/consumers in social, recreational, and living skills activities; record information accurately in the Electronic Health Records system

License/Certificates:
Possession of, or ability to obtain, a valid California driver's license.

DESIRABLE EMPLOYMENT STANDARDS
Volunteer work experience with a Public Mental Health system.

Additional Information

Conditions of Employment: Possession of, or ability to obtain, an appropriate, valid California driver's license. Candidates selected may be required to pass a pre-employment drug and alcohol screening. Additionally, a background investigation will be conducted. Some job classes may also require a physical exam.

Benefit Amount:An annual benefit amount is provided and may be applied towards health insurance premiums (medical, dental, vision, life and long-term disability). This benefit is pro-rated and paid on a pay period basis (24 pay periods).
Health Insurance:A choice of PPO and HMO medical plans which include PPO and HMO dental plans and include dental and vision coverage. Dependent coverage is available. Providers include Anthem Blue Cross, Kaiser Permanente, Delta Dental, and Vision Services Plan (VSP).
Retirement:The retirement plan is a defined benefit plan administered pursuant to the 1937 Act County Employees Retirement Act and integrated with Social Security. In addition to ordinary retirement benefits, the plan provides disability and death benefits. Retirement contributions are made by both the County and the employee. The County has reciprocity with the State of California, contracting PERS agencies, and all County 1937 Act Retirement Systems.
Paid Holiday Leave:12 set days and 1 personal holiday.
Vacation Accrual:
2 weeks per year (0-3 years of service)
3 weeks per year (3-7 years of service)
4 weeks per year (7-11 years of service)
5 weeks per year (11+ years of service)
Limit of 300 hours.
Sick Leave Accrual:12 days per year with unlimited accumulation, 50 hours of which may be used toward family sick leave.
Group Term Life Insurance and AD&D:$10,000; Provided by Standard Insurance Company.
Disability Insurance:Employees are covered by State Disability Insurance. The premium is paid by the employee.
Deferred Compensation:A voluntary deferred compensation plan is available.
