Job Title: Community Health Representative

ESSENTIAL DUTIES
· Organize and conduct community outreach activities focused on chronic conditions like diabetes, heart disease, and obesity.
· Promote a healthy lifestyle to patients and work with the HP staff and clinicians (medical providers and behavioral health staff) to ensure continuum of care.
· Identify health focus areas based on CDC grant program goals and work to achieve those.
· Maintain close working relationship with medical staff and providers to convey information and ensure patient well-being between clinic medical visits.
· Provide reports of patient visits and/or encounters. Collect patient data (vital signs, etc.) as needed and reports all patient encounters in EHR.
· Responsible for meeting CDC grant program goals.
· Participate in the organizational Quality Improvement Program through incident reporting and identifying opportunities for improvement.
· Perform other duties as assigned.
MINIMUM REQUIREMENT
High school diploma or GED. One-year experience in a clinical setting. Clinical competencies updated annually with staff development and training. Previous experience as community outreach worker preferred, along with experience working with patients living with chronic disease- heart disease, obesity, and diabetes. This position is covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes. Must be current with required immunization per the NACA HR Policies and Procedures and Infection Control Policy. BLS and HIPAA Certification. Must pass Level I Fingerprint Clearance and criminal background investigation. Possess a valid State Driver’s License.
KNOWLEDGE, SKILL, AND ABILITIES
· Knowledge of diabetes, obesity, and heart disease to provide the necessary and effective service to patients.
- Knowledge of electronic health records.
· Experience working with Native American clients. Navajo speaking preferred.
- Knowledge of medical terminology.
- Good communication and interpersonal skills to work with staff, supervisor, and key clinical staff.
- Knowledge of Microsoft Office software specifically Outlook, Excel, Word, and Internet.
· Ability to carry out assignments to their completion within expected deadlines.
· Ability to maintain strict confidentiality and comply with HIPAA policies and expectations.
· Ability to prioritize tasks to meet grant and program goals.
· Ability to work independently and with minimal supervision when out in the field- but also must be able to work within a team setting.
