Community Health Navigator - Case Management - Full-Time

Works directly with patients to perform Community Health work - patient health education, patient navigation, and referrals to resources. The Community Health Navigator will work with patients through in-person and virtual appointments to help assess their needs and connect them with resources to close their gaps in health-related social needs. Completion of Community Health worker training is required (provided by employer).

Requirements:

• High school diploma or GED required
• Technical or college degree preferred (e.g. Medical Assistant, social work)
• None required; experience in community service, clinical, medical, or social work environment preferred.
• Bilingual in Spanish is highly preferred.

Responsibilities:

• Working with diverse people, including racial and ethnic minority persons, individuals with variable literacy and education levels,
  persons experiencing poverty, food insecurity, or with other health related social needs
• Delivering health information clearly, explaining terms in plain language, promoting
  health behavior change
• Understanding of the community, including social factors of health, health issues, and ways to improve health
• Identifying needs, strengths, and resources of communities; helping meet needs; helping
  patients understand their needs and overcome barriers; provide social and health support

Schedule:

Full-Time; Monday-Friday

Weekend community events as needed

Pay Range:

Hourly/Non-Exempt

$17.61 - $27.00
