Qualifications Include:
- Bilingual (Preferred)
- Associate degree or equivalent (Preferred)
- Experience with older adults
- Trained Community Health Worker (or certification within 6 mos of hire)
Responsibilities Include:
- Offer health education and evidence-based chronic disease self-management tools to clients and families within the designated area. Enroll those who are at-risk or who have chronic conditions in appropriate HVM programs, such as HEART (Healthy Eating/Active Living/Reduce Stress/Test your Numbers), DEEP (Diabetes Empowerment and Education Program), and other classes.
- Build strong relationships between HealthVisions and community organizations, clinics, social service agencies, and other nonprofits to create an ongoing stream of client service opportunities.
- Develop a list of community resources to refer participants to the appropriate agencies for further health progress. Provide referral information to participants as needed. Follow up with people who have been referred for services to assure that their needs are met.
- Assist the Director in completing the required grant reports in a timely manner. Complete and maintain monthly reports for information and assistance services. Submit reports to the appropriate agencies in a timely manner. Communicate requirements related to documentation and reporting to coworkers as needed.
