This position is situated in the Department of Health and Human Services which uses a person-centered, holistic and integrated approach to collaboratively provide an accessible network of resources that support Montgomery County residents.


Community Connections is the centralized intake, assessment and care coordination for Montgomery County's Department of Health and Human Services. It is home to our highly respected Navicates program.


The Community Connections Supervisor is responsible for the oversight of the coordination and delivery of services performed by the Community Connections staff. Staff are responsible for effectively and efficiently assessing clients for needed county and community services, participating in community outreach, and providing case management and follow-up to ensure individuals are successfully connected to resources. Utilizing exceptional customer service skills, the staff identify, refer and provide monitoring as necessary to ensure the identified needs have been addressed. The Community Connections Supervisor will be expected to fill in as needed and conduct periodic reviews of the staff's work to ensure compliance with the Office's standard operating procedures.

  • Participates in the interview process for all new Community Connections staff;
  • Provides supervision to staff, monitors performance, provides coaching and progressive discipline as necessary; completes monthly supervision, 6 month, and annual evaluations, as well running productivity reports. This includes travel to 3 of the regional offices on a monthly basis at minimum;
  • Troubleshoots around any obstacles clients may have connecting with current/needed services and determining next steps in referring to service providers. May also engage with clients as needed;
  • Oversees Office's Quality Management Plan and conducts monthly auditing of data system to ensure staff are entering data in accordance with the Office's standard operating procedures;
  • Reviews eligibility guidelines for Person-Centered Counseling service (PCC) with Navicates to ensure all eligible clients are being enrolled and documentation is completed accurately. Will liaise with Funder to ensure all enrollments are reimbursed and staff trainings are up-to-date;
  • Through attendance at County collaborative and other meetings, supervisor will gather information on services provided and appropriate contact information and share during team meetings to enhance the knowledge base of Navicates;
  • May need to fill in as a Navicate by engaging clients, providing accurate information and referrals based upon identified needs, or participating in promotional events (including nights &/or weekends);
  • Provides input into the Communications plan which includes development and distribution of all promotional materials, website updates, and social media postings according to the plan;
  • Develops and analyzes quarterly reports for Administrator; information Includes but is not limited to, number of clients served, duration of contacts, provider referrals, type of assistance needed, basic demographic information on client, number of Person-Centered Counseling contacts completed. Analyzes the data to report performance measures and outcomes of the Office;
  • Assists in establishing and ensuring staff are in compliance with standard operating policies of the Office, HHS, and County. This also includes periodic quality assurance reviews as outlined in the Office's CQI plan.
  • Contributes to a positive work environment;
  • Demonstrates excellent customer service skills;
  • Responds to direction from supervisor or other County/HHS Leadership so that the office can fulfill agency's mission;
  • Fills in for the Administrator in the absence of the Community Connections Administrator;
  • Performs other duties as required to maintain office functions.

BA/BS in Human Services, Social Work or related field plus 5+ year's relevant experience (Master's degree preferred); minimum of 1 year supervisory/management in the field of human services experience is required.


Ability to deliver services with an understanding of a wide array of others' personal preferences, choices, values, belief systems, cultural and language differences.


Knowledge of county and community resources. Knowledge of human services delivery system. Experience as a case manager, supervising case management teams, and working in Montgomery County with clients and previous training or certification in trauma informed care and recovery principles are highly desirable.


Experience working with databases, excel, PowerBI, MicroSoft365, virtual platforms and other office software.


Knowledge of exporting data and producing data visualizations.


Flexible, team player who thrives in a collaborative environment. Excellent oral and written communication skills.


Excellent time-management skills.


Ability to work well with community partners and establish ongoing relationships with providers.


Must possess a valid state driver's license as there will be frequent travel throughout Montgomery County.
