Our Community Care Team in Lewiston has a full-time position available for an RN or LPN!

PURPOSE OF POSITION: To collaborate with and coordinate community networks/teams and community-based supports and service providers who share resources to ensure patients can access all the available supports the community has to offer. The ultimate goal is to establish an informed activated patient and a prepared proactive practice team within the context of a supportive and connected community to improve the health of the patient, reduce unnecessary re-hospitalization and impact on health care services. CCT will support this goal by collaborating with the patient the PCMH, and community partners to assure that identified needs are addressed.

EDUCATION: Current RN or LPN License in the State of Maine. Project Manager Certification, and training in Case Management preferred.

EXPERIENCE: Home Care experience preferred, patient care, and Case/Care Management also preferred. Knowledge of community resources

LOCATION: Bangor office with telephonic support to Lewiston, Auburn, Bangor and surrounding towns.
