JOB DESCRIPTION

JOB SUMMARY:

Under the supervision and guidance of a licensed nurse (RN), the CNA is responsible for assisting with direct patient care within the scope of their practice as well as other work on the unit which supports the patient environment. The CNA assists staff to ensure optimal patient care and assists the healthcare team to provide and maintain a clean, safe, and attractive environment for patients. Work will include components of direct patient care, nutrition, observation, documentation, transportation of patients and supplies, and general maintenance of the patient environment. The CNA is a team player.

The CNA will report to the assigned facility manager/supervisor as determined by the contracting facility as well as to the management of Elite Home Care Solutions.

MINIMUM QUALIFICATIONS:

  • Current, unrestricted CNA license/registration
  • CNA licensure/registration in state of practice, as applicable
  • No experience as CNA
  • Current CPR certification
  • Ability to meet all health, compliance and competency requirements
  • The ability to communicate effectively verbally and in writing
  • Other as required for specific assignments or for employment

RESPONSIBLITIES AND FUNCTIONS:

  • Perform patient care as defined by scope of practice — duties may include but not limited to taking vital signs, assisting patients with bathing, eating, dressing, transfers and ambulation, repositioning patients in bed to prevent skin breakdown, change linens, record oral intake and measure urinary output, collect specimens for tests, supply and empty bed pans, interact with patients and family, and transport patients and equipment.
  • Monitor patients and report any variances from normal to the nurse
  • Collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes
  • Demonstrates knowledge of age-specific patient populations and cultural diversity during the provision of patient care.
  • Maintain his/her own clinical and mandatory education requirements and for meeting the established standards of their profession
  • Contributes to the provision of quality care through performance improvement techniques that demonstrate positive outcomes in patient care.
  • Adapt to the needs of the unit by demonstrating flexibility/adaptability.
  • Maintain detailed and accurate records of CNA actions. Document care according to hospital policies and procedures and in a clear, concise and legible manner
  • Demonstrate good infection control practices including hand washing
  • Adhere to all policies and procedures as defined by Elite Home Care Solutions.
  • Assist other staff as necessary to ensure the provision of quality patient care.
  • Maintain a safe and clean environment for patients and co-workers
  • Respond promptly to patient emergencies in a safe and appropriate manner.
  • Keep all appropriate healthcare providers informed of patient condition including changes — includes providing informative yet concise report at change of shift.
  • Maintain confidentiality at all times according to hospital policy and procedures and HIPAA requirements.
  • Act as a patient advocate and provide care in a non-judgmental, nondiscriminatory manner that is sensitive to patient/family diversity while preserving each patient's autonomy, dignity and rights.
  • Report/document all incidents such as errors and patient incidents to agency staff as defined in agency policy and procedures.
  • Report any unexpected patient incidents, including errors, falls, unanticipated deaths, injuries and safety hazards to Elite Home Care Solutions.
  • Assume responsibility to report clinical limitation or the need for assistance to the supervisor.
  • Conduct self in a professional manner at all times — this includes arriving to work on time and completing each shift as determined by the agency.
  • Maintains positive behaviors, approaches, attitude, and commitment to interpersonal service toward patients, visitors, and co-workers.
  • Adhere to facility specific job description for specialty
  • Other duties as defined/assigned by the hospital and assigned supervisor.

OTHER EXPECTATIONS:

  • The agency CNA is an employee/contractor of Elite Home Care Solutions and reports to both Elite Home Care Solutions and the designated supervisor(s) at the agency.
  • First clinical staff will represent Elite Home Care Solutions in a professional, positive manner at all times.
  • Keep Elite Home Care Solutions informed at all times if you are in any way dissatisfied with the assignment you are working. Any issue(s) you are concerned about should first be brought to the attention of the agency supervisor or designee for resolution.
  • If resolution does not occur, you are expected to contact us first to discuss your issues. Elite Home Care Solutions staff will work with you and the facility management to work towards a resolution.
  • All on the job injuries are to be reported to Elite Home Care Solutions.
  • Sick calls will be kept to a minimum. Tardiness will not be tolerated. You will be held to the same expectations regarding excessive sick calls and/or tardiness as the agency employees.
  • Complete & return all required documentation promptly, prior to working first shift for Elite Home Care Solutions and annually as required.
  • Attend agency orientation & other required training as required by the agency.
  • Responsible for maintaining his/her own clinical, licensing, certification, health and/or mandatory education requirements.
  • Comply with all federal, state and local laws and regulations
  • Accept only assignments for which the individual clinician is qualified.
  • Communicate in a respectful manner at all times with Elite Home Care Solutions staff and with management, staff, patients and family of patients of facility.
  • Will not refer a patient to a specific attorney or law firm for legal assistance.
  • Will not discuss any components of compensation with anyone at facility.
  • Follow all assigned agency policy and procedures.
  • Report any security incidents, including property damage to Elite Home Care Solutions.
  • Report any concerns about patient safety and well being to Elite Home Care Solutions.

PHYSICAL DEMANDS:

  • Requires full range of motion including lifting, repositioning, transferring and supporting patients
  • Good eye-hand-foot coordination and good manual dexterity
  • Frequent standing and walking for extended periods of time
  • Ability to push/pull more than 50 lbs, and to lift objects greater than 20 lbs.

Job Types: Full-time, Part-time, PRN

Pay: $16.00 - $19.00 per hour

Standard shift:

  • Day shift
  • Evening shift

Weekly schedule:

  • Monday to Friday
  • Weekends as needed
