Richmond Behavioral Health has a full-time position available for an Enhanced Care CoordinationClinician I/II. Enhanced Care Coordination is an intensive wraparound service in which a skilled team of care providers work together with their clients and family. This position supports, educates, advocates, and motivates clients to enhance their overall well-being. Clients that receive this service are dually diagnosed with both mental health and complex medical needs. The focus of the program is to monitor medical health, ensure attendance and engagement to medical appointments, monitoring medication compliance, linkage to community resources, and providing psychoeducation. The Clinician will coordinate care with both RBHA, Managed Care Providers, and community providers involved in the client's treatment. Partnering with these providers is essential to client success. The overall goal of the program is to provide intensive services to help clients achieve independence in managing their own medical care.

The ECC Clinician is to perform intermediate professional work providing direct clinical service to clients involving intake, assessing client needs, developing treatment plans, providing prevention services, managing cases, collaborating with providers, maintaining records and files, preparing reports, transporting clients to appointments, and related work as apparent or assigned.

Job Duties

Assess and monitor client mental and physical health, medication adherence, and engagement with providers/community resources

Advocate for client medical and mental health needs

Link to community resources/provider, monitor/encourage client engagement

Assist with coordination of appointments and collaborate with providers to ensure continuity of care

Provide and/or coordinate transportation assistance as needed

Provides consultation, prevention services and psychoeducation to clients and their families as needed on a variety of mental and physical health related needs/services to enhance client understanding/knowledge

Collaborate with managed care organization providers and care managers regarding client treatment progress/service needs

Engage in collaborative documentation updating all required documents to remain in compliance

Complete requested MCO documentation, monthly reports, and clinical reviews to capture client progress

Monitors and manages an assigned caseload.

Provides crisis intervention services to individuals and families.

Develops and maintains administrative and clinical files; prepares various reports and case related correspondence.

Participates in clinical, administrative and supervisory meetings; collaborates with other staff members concerning cases.

Performs intakes, client assessments and diagnostic interviews, and develops and writes comprehensive intake and diagnostic summaries and reports; conducts mental health assessment and monitors level of functioning.

Attend client medical appointments as needed, provides physician and staff with updates; maintains medical records in accordance with standards.

Prioritizes medication and lab work monitoring with medical staff to maintain compliance.

Manages and documents contacts; enters and updates information in database.

Coordinates case management and ECC services; coordinates and links client to community resources and medical services; interfaces with other service providers and client support system.

Attends professional conferences and trainings as required.

Supervises student interns as directed (if eligible).

Education and Experience

Master’s degree with coursework in social work, counseling, psychology, or related field and moderate experience working with clients with behavioral health disorders and/or developmental disabilities in a behavioral healthcare setting, or equivalent combination of education and experience.

Qualifying licensed and licensed eligible clinician staff will be eligible for a $4K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.

Special Requirements

Valid driver's license in the Commonwealth of Virginia.
