Job Description
Do you have a passion for helping the citizens of the City of Chesapeake?
Tasks to include:
  • Provides Onsite coverage for ICF-ID; monitors and support individuals as needed.
  • Assesses individuals needs and activities and provide guidance to Direct Support Technicians
  • Support individuals with medications and self-administration of medications.
  • Develops and present trainings for individuals on active treatment/skill-building activities such as personal hygiene, self-advocacy, housekeeping, money management, and cooking/food preparation to promote independence.
  • Participates in an interdisciplinary Personal Support Team (PST) as needed to assist in the development of a plan of support responsive to individuals’ needs. Engages and maintains appropriate contact with family members regarding information on program/individual progress and needs. Confers with medical staff (e.g., medical director, nursing staff)
  • Gathers and maintains information to support periodic and special reports documenting individual progress and activities; maintains records on a daily basis in accordance with policies and procedures.
  • Consults with medical and agency staff and assists in support networking by maintaining communications with day programs, accessing community leisure and recreation facilities and activities in accordance with confidentiality guidelines to discuss individual progress; identify strengths and needs and develop training objectives for person centered plan (PCP).
  • Identifies and defuses crisis situations; evaluates and determines an intervention strategy and contacts necessary program staff in accordance with policy and procedures.
  • Provides transportation for individuals to such places as Doctor’s office/clinic, social activities, and shopping; provide assistance with activities as required.
  • Perform other related duties as assigned.

Required Qualifications
Vocational/Educational Requirement Requires any combination of education and experience equivalent to a bachelor’s degree in social work, psychology, closely related field or other degree as approved by the Virginia Department of Health Professions. Some positions, depending on assignment, may require a degree.
Experience In addition to satisfying the vocational/educational standard, this class requires a minimum of one year of related, full-time equivalent experience. Qualified Mental Health Professional – Eligible (QMHP-E) may be considered in lieu of experience.
Special Certifications and Licenses Depending on assignment, may require a valid driver’s license and a driving record that is in compliance with City Driving Standards. Depending on assignment, may require Qualified Mental Health Professional (QMHP – Adult or Child) or Qualified Developmental Disability Professional (QDDP) certification upon hire; CPR, First Aid, and/or Medication Administration certification within 90 days of hire; and successful completion of Intellectual Disabilities Orientation Workbook and/or Case Management Modules within 30 days of hire.
Special Requirements Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.

Preferred Qualifications
Bachelors degree in human Services, social work, psychological, or closes related field.
Experience working with individuals with Developmental Disabilities.
Working knowledge or experience with using electronic health record systems.
