We’re looking for an experienced RN for part-time Per Diem PRN assignments doing admission assessments, and recertifications in Kent County.

Ideally you will have at least one years of recent experience as a home health RN.

Responsibilities
- Performing in-home Nursing Assessments
- Conducting Client Re-Certifications every 60 days
- Conducting Caregiver Supervisory visits every 60 days
- Manage the client needs and expectations with regards to the services we provide
- Liaising with Case Manager relating to clients as needed
- Providing Plans of Care to Caregivers
Qualifications
- Must be a registered nurse (RN) licensed in the state and have worked as an RN for at least two years
- A minimum of one year of home health care or equivalent experience is required
- Tech Savy: Essential
- Microsoft Office 365, Outlook, Excel, Word.
