JOB SUMMARY

In partnership with the Clinic Administrator, ensures implementation and success of goals, organizes, and coordinates the clinical environment, ensuring efficient and effective delivery of healthcare services in physician practices. Supervises minimum of 10 clinical FTEs and may include LPNs, medical assistants, and other unlicensed personnel. The Licensed Practical Nurse serves as a clinical leader of the healthcare team at UVA Health Northern VA & Culpeper by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to promote optimal health and takes ownership in creating an environment that allows team members to be present with patients and families. The Licensed Practical Nurse utilizes best scientific evidence and compassion to assist the patient in navigating their health journey. The Licensed Practical Nurse accepts responsibility, authority, and accountability for management of clinic tasks in accordance with the current policies and procedures and scope of practice. The Licensed Practical Nurse gives feedback and support to appropriate team members and promote employee and customer satisfaction.

Patient Care: Provides assessment, evaluation, and management of patients.

• Participates in the collection of patient data as needed.
• Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.
• Identifies relevant resources based on necessity and availability.

Communication: Communicates information to others responsible for or involved in the care of the patient.

• Participates in dialogue with the patient/chosen support system to identify the individualized care plan.
• Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care.
• Documents appropriate information in a timely manner.

Teamwork: Participates as part of the interdisciplinary care team in planning individualized care.

• Collaborates with the registered nurse to consider ongoing modification of care plan based on patient’s needs, capabilities, and condition.

Medical Knowledge: Demonstrates core knowledge of biomedical and clinical sciences and the application of this knowledge to patient care.

• Identifies nursing interventions and goals for the patient and reviews goals with the registered nurse.
• Identifies opportunities for process improvement, evidence-based practice, and research.
• Engages in self-reflection and develops personal goals to enhance knowledge, obtain professional certification, or pursue academic degree.
• Integrates gained knowledge into practice.

The incumbent may be asked to perform additional duties as assigned.

QUALIFICATIONS

Education: High School Diploma or GED Required - Graduate of an accredited practical nursing or other healthcare related program required .

Experience: Minimum of 3 years of clinical experience in a physician practice preferred. Minimum of 4 years of supervisory or lead experience required. Refer to the Life Support Training Policy for additional details.

Licensure: Applicable state LPN license required. BLS required.

Additional Skills/Requirements Required : Excellent human relation skills, possess effective oral and written communication skills, possess leadership and problem-solving skills, operate personal computer. Ability to successfully complete Leader Education and training.
