Job description

The primary purpose of this position is the monitoring of PDN/PCA nursing and nursing support staff to ensure consistently high and across-the-board clinical performance. The clinical manager is responsible for the development and consistent application of organizational clinical processes to ensure client satisfaction. This position serves as the first line of supervisory response to nurse issues, and client complaints.

Also ensures the that all plans of care operate with established goals, implementation, monitoring and evaluation of assisgned patient case loads in order to provide quality care appropriate to the clinical needs of GenCare clients. Essential Responsibilities and Duties include the following (Others may be assigned): ~ Provide department-specific orientation to all new PDN/PCA clinical staff. ~ Assist conducting interviews if needed in conjunction with Director of Nursing (DON).

~ Assist the Director in evaluating and resolving employee complaints, grievances, and turnover issues. ~ Performs daily chart audits. ~ Assure appropriate staff supervision during operating hours.

~ Ensure staff development including orientation, in-service education, continuing education, and evaluation of staff in conjunction with DON. ~ Develops standards that ensure safe and effective services to patients and families. ~ Establishes and maintains effective channels of communication.

~ Consults with physicians in matters relating to patient care services. ~ Assist with the orientation of new professional staff to familiarize them with GenCare policies and procedures, and report training hours to DON and HR for payment. ~ Review nurses' schedule for recertification, discharge, and aide supervisory visits.

~ Reviews weekly schedule submitted by nurses for changes or updates. ~ Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff. ~ Provide daily supervision of district nurses and Home Health Aides to ensure the quality of services and maintenance of standards.

~ Coordinate and schedule the daily workload of each county utilizing GenCare personnel to their fullest capacity. ~ Assess staff performance during their probationary period and annually with the Director of Nursing. ~ See patients in the home setting as needed.

~ Maintain nurse and client email lists to ensure successful DON survey contact. ~ Ensure the proper supply and distribution of personnel protective equipment for field staff. ~ Participate in the annual planning of the Mandatory Meeting curriculum.

~ Educate patient and family (or designee) to processes, care practices and GenCare procedure as needed. ~ All other duties as assigned. ~ Data entered here will feed to the Position Announcement and Evaluation sheets Qualifications, Education, Experience: ~ Nursing diploma or associates degree in nursing required.

~ Bachelors degree in nursing or related field preferred. ~ Three (3) to five (5) years of related clinical experience required, with at least 1 year of pediatric clinical experience. ~ Data entered here will feed to the Position Announcement form Knowledge, Skills & Abilities: ~ Knowledge of nursing educational assessments and learning strategies ~ Knowledge of OASIS preferred ~ Knowledge of Patient Driven Goals Model (PDGM) preferred ~ This position has access to patient medical and financial records, thus must adhere to all federal, state, local, and organizational privacy laws, guidelines, and requirements.

~ Equipment Requirements: Computer, mobile telephone, laptop, office multiline phone, general office equipment. ~ Physical Requirements: Sitting, standing, stooping, walking, talking in-person, talking via telephone, minimal lifting (15 lbs.). Metrics Responsible For: Weekly Metric: Quality assurance procedures run on each Start of Care (SOC) within 48 hours = 100% Weekly Metric: Achieve and maintain orders completed = 100% Monthly Metric: Monthly case conference bringing, with outstanding issues elevated to DON attention = 100% Weekly Metric: Admission of all PDN cases = 100% Monthly Metric: Attain and maintain 100% QA Delivery (via Survey Monkey to all clients), and sattain <90% score average on PDN/PCA surveys.
