JOB TITLE: Medical Records Coder and Clinical Scheduler

DEPARTMENT: Coble Creek

HOURS: 8:30am - 4:30pm Monday - Friday

REPORTS TO: Director of Nursing & Administrator

POSITION SUMMARY: Primary responsibilities are performing detailed review of medical records, identifies and adds pertinent diagnoses and procedures codes at admission and throughout long term stay, assists with collecting information to file insurance and abstracts/verifies resident information into computer database. Primary scheduler responsibilities are coordinating day to day scheduling verifying schedules are set for each shift within facility.

Medical Records Coder Essential Duties and Responsibilities:

1. Review medical record documentation and accurately assign codes for the primary/secondary diagnoses and procedures using ICD-10-CM, ICD-10-PCS with knowledge of reimbursement reporting requirements.
2. Knowledge of PDPM (Patient Driven Payment Model) for Medicare Part A Skilled Nursing Facility Payment System. Ability to establish strategies for transitioning from RUG-IV to PDPM.
3. Maintain a thorough understanding of anatomy and physiology, medical terminology, pharmacology, disease processes and surgical techniques through participation in continuing education programs to effectively apply ICD-10-CM coding.
4. Collect information to fulfill insurance requests for resident payment throughout stay.
5. Uses Point Click Care, MS Office and other appropriate software to compile and validate medical information.

Medical Scheduler Essential Duties and Responsibilities:

1. Responsible for coordinating day to day schedule for both Nurses and CNA’s, and verifies that schedules are set for each shift within facility.
2. Must be creative with completing the schedule and filling in all open shifts
3. Maintains pertinent information regarding availability and current contact information.
4. Documents call outs and updates the staffing sheets with revisions as necessary
5. Coordinates, reports and posts schedules of hourly staff.
6. Maintains appropriate staffing ratios and monthly nursing schedules
7. Response to requests for Paid Annual Leave
8. Communicates with Shift Supervisor, DON and Administrator regarding schedule changes needed for training and in-services
9. Notifies appropriate personnel of call-ins and assists with securing coverage.
10. Maintains accurate attendance records and notifies management of attendance-related issues.
11. Accurately enters and maintains data relative to scheduling using company software programs.

May be required to perform other duties in keeping with the nature of this position which will not significantly alter the job requirements

Education and/or Experience:

• Two (2) or more years of coding experience highly preferred.
• Must have experience working in Skilled Nursing or Long Term Care environment.

Certificates, licenses, registrations:

• Prefer North Carolina Certified Nurse Aide One certification.
• Prefer one of the following active/current certifications:
  • Registered Health Information Administrator (RHIA)
  • Registered Health Information Technician (RHIT) with Certified Coding Specialist (CCS)
  • American Health Information Management Association Certification (AHIMA) with Certified Coding Specialist (CCS)
  • American Academy of Professional Coders Certification (AAPC) – Certified Professional Coder (CPC)
