Clinical Quality Improvement Coordinator

Position Information: The Care Management Clinical Quality Improvement Coordinator is responsible for monitoring the care management case assignment data, assist with quality reviews of assignment criteria and implement report changes, assigning cases, and chart reviews as requested. Plans, coordinates and facilitates the Monthly Lunch and Learn series, including monitoring attendance, securing speakers, CEU distribution, and maintaining the master distribution list. Provide data and quality support for Sooner HAN care management services and Sooner HAN leadership staff.

Essential Duties:

  • Participate in continuous Quality Improvement efforts, including providing feedback on process changes and updates, chart audits, closure reviews, and other care management activities.
  • Provide direct program support for Sooner HAN Care Management services by performing various clerical and office duties such as maintaining databases, preparing for events, preparing monthly reports, and providing quality support to the care management team.
  • Maintain professional and timely communication with the Oklahoma Health Care Authority by sending critical information to, receiving updates from, and following up on information communicated by the Oklahoma Health Care Authority.
  • Coordinates surveys/evaluations in support of the goals of the lunch and learn series.
  • Prepare monthly reports and update reports on an ongoing and timely basis.
  • Prepare for internal and external meetings.
  • Maintain and regularly update lists of professional contacts.
  • Plan and prepare for lecture series and events hosted by the Sooner Health Access Network.
  • Communicate important events and information to professionals outside of the Sooner Health Access Network.
  • Maintain professional communication with care managers, healthcare providers, nurses, and social workers in clinics across the state.
  • Maintain professional follow up with care managers, healthcare providers, nurses, and social workers in clinics to requests for information and assistance.
  • Utilize database management, reporting, and information systems to gather data, organize reports, and identify individuals eligible for care management.
  • Participate in regular case staffing meetings and reviews.
  • Provide quality support as needed to the care management staff, supervisory staff, and department faculty.
  • Other duties as assigned.

Qualifications:

Education: Required: Bachelor's Degree in Nursing or other Allied Health program, AND: 

  • 24 months clinical health care experience in any of the following: direct patient care, medical record and clinic/site reviews, patient complaint investigator, clinical guidelines, and quality of care/risk management/patient safety issues.

Working Conditions:

  • Physical: Sitting for long periods of time. Speaking, walking, and reaching.
  • Environmental: Office Environment.

Knowledge, Skills & Abilities:

  • Applicant must possess knowledge of computer applications, data management and/or analysis.
  • Proficiency in the use of Personal Computers, including MS Word, MS Excel, MS Access (or other similar database), MS Outlook, Adobe Acrobat, MS PowerPoint. 
  • Experience with healthcare documentation systems. 
  • Strong communication skills (both oral and written).
  • Strong organizational abilities.
