Remote/Virtual position, upon hire need to work in Washington State. There will be some travel to clinic locations when needed throughout the Puget Sound.
The Clinical Pharmacist Practitioner (CPP) will collaborate with primary and specialty care physicians and staff to provide comprehensive, chronic disease state management with an emphasis on optimizing medication therapy for Swedish Medical Group (SMG) patients.

Required qualifications:
- PharmD from an accredited school of pharmacy.
- Upon hire: Washington Pharmacist License
- A post-graduate year residency (PGY1) or a minimum 2 years’ experience in area of specialization involving direct, collaborative patient care, in lieu of residency.
- Hands-on experience in relevant patient assessment, medication adjustment, monitoring, and all other aspects of patient care.
- Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
- Experience precepting students and/or residents.
Preferred qualifications:
- Within 2 years of hire: Board Certified in area of specialization.
- A post-graduate year 2 residency (PGY2) or a minimum 3 years’ experience in area of specialization in lieu of residency.
