Clinical Pharmacist - Transplant Team

A healthcare network in New York City is currently seeking a licensed Pharmacist for a great Clinical position with their growing Transplant team in the Bronx.
About the Opportunity:

• Schedule: Monday to Friday
• Hours: 7:30am to 4pm
• Department: Transplant Surgery

Responsibilities:
The Clinical Pharmacist will be responsible for:

• Providing care to patients on the kidney, liver and pancreas transplant services
• Effectively enhancing the drug therapy outcomes of abdominal transplant patients, accomplished through continual assessment of the appropriateness, safety and efficacy of individualized transplant patient pharmacotherapy
• Providing recommendations, information, and education to medical/surgical, nursing, and pharmacy personnel and other healthcare professional and be involved in clinical research in transplantation or other related disciplines
• Performing other duties, as needed

Qualifications:

• 3+ years of experience in solid Organ Transplant Pharmacotherapy
• PharmD
• NYS Pharmacist Licensure
• Completion of a PGY2 in Solid Organ Transplantation OR an ASHP-accredited PGY1 Pharmacy residency
• Eligible for Board Certification in Pharmacotherapy (BCPS) or Solid Organ Transplantation (BCTXP)
• Eligible to be credentialed as an Allied Health Professional
