Job Description

PURPOSE OF POSITION:

To establish clinical practice which consists of participation in patient care rounds on pediatric inpatient service.

To support decision making regarding drug prescribing, monitoring, and regimen adjustments, therapeutic drug monitoring, nutrition support, as well as solving patient- and drug-related problems.

To act as a liaison between pharmacy, nursing, and the medical staff to educate on current drug-related procedures, policies, and guidelines.

To identify ways to optimize drug cost through appropriate use of drugs in the pediatric care units and through implementation of cost-containment measures.

To participate in quality assurance programs to enhance safety and efficacy of medication use.

To collaborate with nursing, medical staff, and hospital administration to prepare the pediatric service for the Joint Commission on the Accreditation of Healthcare Organizations survey and responds to any deficiencies identified.

AREAS OF RESPONSIBILITIES:

Round as a member of the interdisciplinary critical care team to provide comprehensive medication management for all Pediatric patients.

Provide pharmacokinetic monitoring and therapeutic adjustments when a targeted drug is prescribed.

Prospectively evaluate all drug therapy for appropriate indication, dose, drug interactions, drug allergies, and monitor the patient's pharmacotherapeutic regimen for effectiveness and adverse drug events (ADEs), and intervene as needed.

Develop, enforce, and maintain guidelines, policies, and order sets that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate therapy

Ensure medication errors and ADRs are reported and pathways developed to prevent future occurrences.

Support of quality improvement projects and research initiatives.

Performance of medication use reviews to assess cost effectiveness and medication appropriateness.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

Develops or participates in a clinical practice by interacting with physicians, nurses, other health care professionals, and patients as a member of an interdisciplinary team on patient

care units to guide and recommend appropriate medication use.

Participates in focused clinical initiatives as defined by the department (e.g. pharmacokinetic consultation, antimicrobial stewardship, etc.).

Participates in Medication Use Evaluations when appropriate.

Monitors assigned patient populations for clinical progress including renal/hepatic function, culture results, and other applicable laboratory tests. Recommends adjustments to therapy as necessary. Participates in education of other Pharmacy staff and other healthcare professionals.

Focus on cost avoidance and cost savings due to medication use

Participate in the development of medication management programs within the institution, including clinical guidelines, critical pathways, disease management, and drug use programs.

Participate in providing didactic and experiential training in Clinical Pharmacy for PharmD students Participate in the professional development and competency of Clinical Pharmacy staff.

Function as an educational resource for pharmacy staff.

Operates and troubleshoots automated dispensing systems and assists in the monthly inspection of the patient care units, as needed.

Documents interventions, adverse drug reactions (ADRs), and medication errors.

In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function.

Maintains responsibility for both the clinical and the distributive activities of the Pharmacy Department as necessary

Assists with staffing as deemed necessary.

Perform other related duties as required.

Minimum Qualifications

For Level I:

1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:

1. Two years of post licensure experience; or

2. Completion of a post graduate specialty residency or fellowship program; or

3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:

1. One year of post licensure supervisory experience or specialty clinical experience.
