Clinical Pharmacist Specialist (BMT, Leukemia, & General Oncology) 

Oncology Clinical Pharmacist Specialist – General Oncology

The Clinical Pharmacist Specialist is an integral member of the oncology team and supports the multiple oncology practices within the institution. The pharmacist provides a variety of important services to both the multidisciplinary team and the patient and family members. Services provided by the Clinical Pharmacist include, but are not limited to, the following:

  • Oversee the pharmacological management of oncology inpatients.
  • Evaluate the appropriateness and effectiveness of medications, with an emphasis on oncology treatment plans.
  • Review inpatient medication profiles for appropriateness of therapy and opportunities for optimization.
  • Participate in daily inpatient interdisciplinary rounding.
  • Manage the chemotherapy processes of the infusion pharmacy satellite.
  • Assist with treatment plan review and modifications.
  • Provide therapeutic drug monitoring for immunosuppressants, antimicrobials, anti-seizure medications, anticoagulants, etc.
  • Manage antimicrobial therapies and promote antimicrobial stewardship in both prophylaxis and treatment recommendations.
  • Assist in supportive care management, including pain/palliative care.
  • Assist with chemotherapy drug shortage mitigation planning.
  • Participate in the Pharmacy & Therapeutics Committee
  • Perform Medication Use Evaluations (MUEs) and formulary reviews.
  • Collaborate with other healthcare professionals to develop order sets, guidelines, policies, and protocols to promote safe and streamlined patient care.
  • Contribute to institutional and collaborative research and scholarly activities.
  • Provide education to the healthcare team including pharmacists, nurses, physicians, fellows, and residents through in-services, presentations, lectures, clinical pearl memos, and competencies.
  • Preceptor PGY-1 pharmacy residents during their oncology learning experience

 

Qualifications

Required:

  • Must be a NJ Licensed Pharmacist with an active license.
  • PGY2 Oncology Practice Residency, or 5 years clinical oncology practice experience

Preferred:

  • Practice experience in an Oncology inpatient setting
  • Board Certified Oncology Pharmacist (BCOP)

 

Oncology Clinical Pharmacist Specialist – BMT & Leukemia

The Clinical Pharmacist Specialist is an integral member of the hematopoietic stem cell transplant/cellular therapy (HSCT/CT) service. The pharmacist provides a variety of important services to both the multidisciplinary team and the patient and family members. Services provided by the Clinical Pharmacist include, but are not limited to, the following items:

  • Oversee the pharmacological management of patients in our Bone Marrow Transplant program, inpatient and outpatient.
  • Evaluate the appropriateness and effectiveness of medications, with an emphasis on reviewing oncology treatment plans - including order review for mobilization and conditioning chemotherapy.
  • Managing the chemotherapy processes and assistance with treatment planning and review as well as preparation of order sets and policy documents
  • Participation in daily inpatient interdisciplinary rounding
  • Thorough patient medication profile review
  • Provide therapeutic drug monitoring for immunosuppressants, antimicrobials, busulfan, etc.
  • Provide education to the healthcare team including pharmacists, nurses, physicians, and residents through in-services, presentations, lectures, clinical pearl memos, and competencies.
  • Assist in supportive care management including pain/palliative care.
  • Optimize graft-versus-host-disease prophylaxis and management.
  • Manage antimicrobial therapies and promote antimicrobial stewardship in both prophylaxis and treatment recommendations.
  • Assist with policy and guidelines development. including participation in the Pharmacy & Therapeutics Committee
  • Assist with chemotherapy drug shortage mitigation planning.
  • Perform Medication Use Evaluations (MUEs) and formulary reviews.
  • Contribute to institutional and collaborative research and scholarly activities.

The role of the pharmacist outlined above are all in accordance with recommendations from the American Society for Blood and Marrow Transplantation, the Foundation for the Accreditation of Cellular Therapy (FACT), and the Joint Accreditation Committee ISCT-EBMT (JACIE).

 

Qualifications
