The Job

The Clinical Pharmacist, Care Management will perform comprehensive and targeted medication management in high risk plan members to identify, resolve, and prevent medication-related problems to improve care outcomes.


You Will

Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution.

Complete comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities.

Conduct engaging and comprehensive telephonic medication consultations with members and delivers high quality evidence-based therapeutic recommendations to providers.

Utilize prescription and medical claim data to generate clinical recommendations for medication management.

Complete pharmacotherapy assessment of patients' claims, past medical history, and disease states.

Collaborate with all members of the patient care team by actively communicating and reporting pertinent patient care information and data in a comprehensive manner.

Provide medication education to patients to improve medication adherence. Monitor of complex comorbidities (DM, HTN, hyperlipidemia, bone disease, ID, etc.).

See the whole person, take into account Social Determinants of Health, and connect members to benefits as applicable.

Perform job duties with sensitivity to the patients’ diverse cultural backgrounds.

Navigate the Part B and Part D benefits to support member care.

Research and maintain knowledge of current and emerging clinical trends. Takes initiative to continually learn and maintain clinical and regulatory knowledge.

Actively participate in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information.

Maintain competency skills to identify medication safety concerns in the age-related population and institute additional measures to provide a safe environment with consideration to the developmental changes in the patient population.

Ensure projects/tasks, as assigned, are completed in a timely manner and contribute to success of the team.

Serve as preceptor to pharmacy students and residents as assigned.

Actively support the achievement of SCAN’s Vision and Goals.

Other duties as assigned.


Your Qualifications

Pharm.D. or equivalent degree from an accredited school of pharmacy in the USA required.

Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience preferred.

Current pharmacist licensure in the State of California.

Board Certification preferred (Geriatric, AmCare, or Pharmacotherapy)

Certified Diabetes Care and Education Specialist (CDCES)/Certified Diabetes Educator (CDE) preferred

BILINGUAL - Strongly Preferred, bilingual in English/Spanish. (Test will be administered to assess proficiency.)

Managed care experience with knowledge of CMS Part D regulations preferred.

Experience in palliative care preferred.

Dynamic and confident person with strong clinical skills and a mind that is both flexible and capable of critical thinking.

Strong communication skills and the ability to build positive, trusting relationships with providers and other health care staff.

Must be highly detail oriented, motivated, and independent.

Comprehensive knowledge of medical terminology as related to drug usage.

Comprehensive knowledge of common disease states and pathologies.

Ability to comprehend patient medical management plans.

Ability to assess and apply current pharmacological and biopharmaceutical principles for the selection and use of drug products in a clinical setting.

Ability to extract pertinent information from the patient or other sources and relate it to the patient care plan.

Possess the verbal and written communication skills required to fulfill the pharmacist’s responsibilities.

Ability to provide recommendations and monitor the use of drugs.

Knowledge of laws, regulations and standards relative to pharmacy practice.

Ability to adapt to change.

Ability to prioritize workflow.

Ability to evaluate medication regimens to meet the therapeutic needs of patients.

Ability to learn and adapt to the use of automated information systems in daily practice.

Demonstrated competency in techniques of pharmaceutical calculations, compounding and pharmacokinetics.

Possess patient-centered communication skills

Possess computer skills necessary to perform job functions.


PREFERRED QUALIFICATIONS

Knowledge and experience with organizing quality improvement initiatives and presenting findings to leadership.

Previous experience in precepting and mentoring pharmacy students and residents is desirable.
