The Clinical Pharmacist Specialist – Infectious Disease is responsible for the provision of safe, effective, appropriate and cost-efficient medication therapy to patients in the area of Infectious Disease. Under direction of the Chief Pharmacy Officer he/she will assist in the planning, organizing and monitoring of the delivery of pharmacy services for all of Southern California Region.

- Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients receiving antimicrobials. Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in Infectious Disease through improved reporting, analysis and follow-up.
- Oversees daily prospective audit of antimicrobials. Monitors patients in a specific unit/specialty service with includes collecting patient-specific data, interviewing patients, and identifying patient goals of therapy. Working with the Infectious Disease service to evaluate appropriate use of restricted antimicrobials. Compiles data and metrics on antimicrobial use include spend trend.
- Working with and co-chairing the Antimicrobial Stewardship committees across the system to maintain and update guidelines. Reporting data and findings to the Antimicrobial Stewardship, Infectious Control and Pharmacy & Therapeutics Committees. Coordinating, facilitating, and promoting education, training, competency assessment and performance improvement of pharmacists and other healthcare professionals in the appropriate use of antimicrobials. Attends respective meetings at all Alta sites and travels to site on a regular base.
- Documents clinical interventions in a timely and professional manner. Monitors and reports drug errors, adverse drug reactions, allergies, and patient adherence issues .
Required Qualifications
- Three to five years in hospital practice emphasizing clinical pharmacy, pharmacokinetics and drug information services
- Licensed by the CA State Board of Pharmacy
- Graduate of an accredited School of Pharmacy recognized by the California State Board of Pharmacy
- Ability to maintain flexibility and composure in a constantly changing environment
- Ability to read, write and communicate effectively in English
- Strong time management skills and ability to effectively prioritize tasks
- Ability to establish and maintain effective working relationships within the organization.
Preferred Qualifications
- Previous experience as a Clinical Pharmacist
- Doctor of Pharmacy and pharmacy residency (PGY2 ID)
