Schedule Details: This is a Full Time, benefit eligible position Monday through Friday, 8:00am-4:30pm. Weekend and holiday rotation.
Must have at least 2 years of RN experience in med surg, home health or HCU with experience with COPD, heart failure, post partum hypertension.
This position will work 100% on site

What You Will Do:
The Telemonitoring RN monitors patients’ vital signs/trends and provides appropriate triage and follow-up with physicians and other care team members. There are two main goals that the Telemonitoring RN assists with: daily remote monitoring to identify true changes in condition and provide appropriate early intervention while successfully educating the patient. This education is done in a comprehensive teaching method to ensure that the patient is able to independently manage their health after discharge from the Telemonitoring program.
As an integral part of the Telemonitoring Program, this position has a high level of responsibility and therefore requires an individual who is independent, communicates well with other staff members and physicians, and is a respected and trusted member of the care team.
What You Will Need:
- Associate's Degree in Nursing required. Bachelor’s Degree is preferred
- Wisconsin Registered Nurse License
- Valid American Heart (AHA) Basic Life Support ( BLS) certification
- Minimum of 2 years of experience as RN (4+ years preferred)
- Previous experience in Med/Srg, Heart Care or home health strongly preferred
- Triage skills strongly preferred (Medical Group, Clinic setting)
Additional Required Skills:
- Strong clinical judgment and patient triaging skills are vital
- Strong education skills with motivational interviewing skills
- Exceptional patient assessment (telephonic assessment) skills
- Efficient time management with minimal oversight
- Strong phone and written communication skills
