Pay Range: $39.00-43.00 an hour*

Position Type: Full-time, First Shift
Essential Duties of a Clinical Coordinator Nurse, RN:
- Coordinate and schedule healthcare screenings for patients, ensuring timely appointments and adherence to recommended screening protocol; address any possible barriers to patient appointment
- Perform telephone triage of patients by accurately assessing symptoms based on subjective and objective findings
- Navigate patients through complex healthcare systems, ensuring timely access to services and addressing logistical challenges
- Performs physical, psychological, and social assessment of patients
- Serves as a liaison connecting healthcare providers and patients through comprehensive assessment of patient needs and effective communication of pertinent health information
- Evaluates the effectiveness of care delivered, ensuring adherence to quality standards and facilitating seamless care coordination across all aspects of the patient's healthcare journey
- Provide health education to patients, individuals and their families, facilitating informed-decision making and empowering them to actively participate in their care
- Participate in interdisciplinary meetings committees on meetings focused on quality improvement, contributing insights and recommendations to enhance patient care processes and outcomes
- Collaborate with healthcare team members to develop and implement to develop care plans that are tailored to meet the individual needs and preferences of patients
- Administer medications/treatments as ordered by provider and complete prior authorizations for medication and diagnostic services
- Maintain safe, secure, and healthy work environment by establishing and following standards and procedures, in accordance with internal and legal regulations
- Assist with examination and in-office treatment by handing providers instruments and materials
- Deliver exemplary customer service in order to provide a positive patient experience across the organization
Minimum Qualifications:
- Current New York State Registration as a Professional Registered Nurse
- Completion of a Bachelor's degree or higher from an accredited school of nursing.
- 5 years of experience as a Registered Nurse, Medical/Surgical Hospital Experience Preferred
- BLS Certification required
- Knowledge in cancer screening guidelines and procedures, risk assessment, patient education, and informed decision making
- Strong communication skills, cultural competency, clerical skill including documentation, and staying updated with advancements in cancer screening practices.
