Position Summary:
We have an exciting opportunity to join our team as a Clinical Nurse Coordinator - Pediatrics/Bone Marrow Transplant.
In this role, the successful candidate The pediatric bone Marrow Transplant (BMT) coordinator works with the pediatric and adult bone Marrow Transplant team in the planning and execution of the comprehensive care of pediatric transplant candidates and potential donors during the pre-transplant, intra-transplant periods and post-transplant intervals, in both the outpatient and inpatient settings. The coordinator role will be key in helping the patient and family navigate the medical system fostering an environment that promotes patient/family satisfaction and patient safety.
Job Responsibilities:
- Initial bone Marrow Transplant consultation
Pediatric BMT attending will determine if a patient is a transplant candidate and will communicate with the pediatric BMT coordinator to meet the patient and family in a shared initial visit, including the primary BMT attending and other team members who will be participating in the care of the patient.
- Care of the bone Marrow Transplant candidate
Pre-Transplant
- Work closely with BMT team to help prepare the patient to the upcoming admission.
- Coordinator will be responsible for initiating, preparing and completing the pre-transplant checklist and forwarding it to the primary attending for review and finalization before admission and transplant education for the patient and family.
- Coordinator will be responsible for initiating insurance authorization of transplant work up and admission in collaboration with the finance department.
- Coordinate pre-transplant assessments with:
- Social Work
- Child Life Specialists
- Nutritionist
- Physical Therapy
- Dental
- Pediatric subspecialty services
Admission
- Alert PICU team about upcoming admissions the week prior admission and send a patient specific presentation (made by the BMT attending) to the PICU team with the details on the transplant plan.
- Work with PICU team and PharmD to ensure orders/meds are in EPIC and coordinate with inpatient RN for timely administration of all medications, IV fluids, and supportive care.
- Provide education to the nursing staff to assure clear understanding of the timing of infusions/plan of care each day for cytoreduction/myeloablation.
- Ongoing education, including infection control principles, low microbial diet and daily expectations regarding inpatient care (i.e. vital signs frequency, daily weights, line care, showers).
- Ensure that consulting services for Psych, PT/OT, Nutrition, Child Life and School Teacher are in place for the patient.
During Transplant
- Coordinate stem cell infusions with appropriate teams.
- Work with BMT attending/PAs/fellows/lab supervisors to order drug levels/ Chimerism/ Immunophenotyping/ viral surveillance and other BMT specific labs that are drawn by RN.
- Schedule weekly meetings/check-in with caregiver to help identify needs and psychosocial support.
Discharge Education and Planning
- Collaborate with BMT attending, PICU and PharmD, patient and patient caregiver to determine goals for discharge, medications and supplies needed for home to ensure all prescriptions have been ordered and filled prior to discharge.
- Review discharge medications and instructions with family prior to discharge along with PharmD.
- Collaborate with social worker and case manager to ensure all discharge teaching has been completed and patient/family are able to care for themselves at home, have home care/services in place, initiate and assist with completion of Letters of Medical Necessity, school forms, FMLA, etc.
- Communicate with members of the psychosocial team to ensure patient's needs are being met and a follow-up plan outpatient is in place.
- Collaborate with Pediatric Hematology-Oncology fellow to send discharge email, including details of initial outpatient appointment and needs.
- Communicate with required consultants in the outpatient setting to ensure appropriate follow-up.
Outpatient Follow-up
- Continue to assist the BMT team on post-transplant follow up care including ongoing medication education and reconciliation.
- Coordination and communication with related services for post-transplant specific evaluations and procedures.
- Care of the bone Marrow Transplant related donor
Donor evaluation
- Work closely with BMT team to help prepare the pediatric related donor for the upcoming stem cell collection.
- Coordinator will be responsible for initiating, preparing and completing the donor checklist.
Stem Cell Collection
- In coordination with the BMT donor physician and appropriate teams, the BMT coordinator will be responsible for scheduling and planning of the leukapheresis for peripheral blood stem cell donors (i.e. autologous and pediatric related donor), including education on mobilization regimen, line placement, communication with apheresis team, and cell processing orders are in place.
- For donors who will undergo bone marrow harvest, the BMT coordinator will work with BMT attending and appropriate teams to ensure OR time is scheduled, harvest supplies are available, stem cell lab is available.
- Additional Responsibilities:
- Assist with clinical trials and follow-up with data collection.
- Triage and respond to patient communication within the practice policy.
- Work collaboratively with nursing leadership to ensure timely communication and education of the nursing staff about matters relating to transplant patient population.
- Facilitate the continuity of care across the continuum of services, e.g. homecare, other facilities, appointments with physicians and other clinicians.
- Attend biweekly pediatric and adult combined meetings which are dedicated to patient management, quality improvement and education.
Minimum Qualifications:
To qualify you must have a Minimum Qualifications:Graduate of an accredited Nursing Program. Current New York State licensure. BS degree in nursing. 3-5 years progressive leadership experience in a large acute care facility. The clinical Coordinator must possess a comprehensive knowledge of the functions of Operating Room Services. It requires clinical knowledge of the responsible assigned services. She/he should have completed a rotation through her specialty areas and demonstrate a working knowledge of same. The Clinical Coordinator must demonstrate the ability to teach concepts of the basic sciences, interpersonal relationships and team nursing. She/he must possess leadership and organizational qualities, an understanding of leadership principles, group dynamics and skill analysis. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
