As a Clinical Coordinator, RN for our Intensive Care Unit you will be a vital member of a dynamic team responsible for responsible for the complex, multi-disciplinary coordination of specialty patient population(s), developing, evaluating, and revising care pathways based on assessments conducted through engagement with the patients and families in the ICU. Summary of Responsibilities:
- Accountability for the coordination of nursing care, including direct patient care, patient/family education and transitions of care
- Support professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family
- Educates and serves as an expert resource for other personnel
- Contribute to the strategic planning process and attainment of goals of the organization
- Function within the Day Kimball model, which includes accountability for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients
- Provide leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts
- Delegate patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources
- Mentor and coaching to nursing staff; and may participate on committees or projects
Knowledge, Skills, and Abilities:
- Ability to recognize unsafe and life-threatening situations and how to intervene appropriately
- Demonstrated leadership skills in critical thinking, effective communication, organizational skills, problem solving, and ability to make appropriate independent decisions
- Working knowledge of applicable Standards of Practice
- Computer skills required, prior experience with electronic medical record systems preferred
- Demonstrated compassionate customer service and ability to meet the needs and expectations of patients and healthcare colleagues
- Solid foundation in evaluating plans of care and reformulating as necessary
Qualifications:
- Graduate of an accredited school of nursing is required; Bachelor of Science in Nursing is preferred
- Registered Nurse (RN) license issued by the state of Connecticut is required
- A minimum of five (5) years of experience as a Registered Nurse (RN)
- Three (3) years of experience in critical care is preferred
- A minimum of one (1) year of experience in a leadership role is strongly preferred
- American Heart Association Healthcare Provider CPR certification or equivalent is required; if not currently certified, must obtain within three (3) months of employment
- Additional specialty certification/training as required by the work area
