UConn Health is looking for a highly skilled RN Clinical Case Manager with experience in acute healthcare. This role is to engage the referral process to UHIPS for home infusions services. Coordinate service and patients at the time of discharge. This position is key to transition of care to an off campus administration of IV therapies.

SCHEDULE: 40 hours per week, primarily Monday-Friday 8:00 a.m. to 4:30 p.m., flexibility to cover days and evenings, weekends and holidays as assigned to support department needs. 30 minute unpaid meal break
MINIMUM QUALIFICATIONS, KNOWLEDGE, SKILLS, ABILITIES:
Knowledge of the principles and practices of case management.
Knowledge of: various disease modalities and expected and adverse outcomes of therapeutic interventions.
Knowledge of standards of practice for interdisciplinary delivery of patient care,
Excellent interpersonal and oral and written communication skills.
Problem solving skills and the ability to establish priorities and manage effectively;
Ability to elicit relevant information and cooperation of patients and staff, and to adapt teaching methods to individual needs.

EXPERIENCE AND TRAINING
GENERAL EXPERIENCE: Master's degree in Nursing
SUBSTITUITION ALLOWED: Bachelor's degree in nursing with three years' experience in an inpatient hospital setting or Case Management experience.
SPECIAL REQUIREMENTS:
Incumbents in this class must maintain licensure as a registered nurse
Minimum of 2 years of clinical nursing experience in an acute care setting (Bachelors’ degree needs 3 years of experience)
PREFERRED REQUIREMENTS:
Acute Care and Home care experience.
EXAMPLES OF DUTIES:
Assess patient/family clinical status needs related to medical diagnosis, treatment plan, treatment providers, treatment options, financial resources and discharge planning needs.
Interprets critical data and makes recommendations to effect improvement in outcomes based on trends in inappropriate, inefficient or costly utilization.

Monitors critical aspects of care affecting reimbursement.
Provides point of contact for community physicians, pharmacy, staff, payers and patient regarding plan of care and questions related to discharge and resource utilization.
Monitors case load, collecting data and analyze outcome data (timely starts and end case, phlebitis- infection rates, IV attempts, infiltrates medication reaction and other outcomes as programs develops).
Responsible for maintaining and growing core therapy admits through customer relationships and providing responsive customer service with assigned accounts
Accountable for independently providing and collaborating with Case Managers, payers, physicians, clinicians, referral sources, home health agencies and teams to facilitate patient discharge and coordination of care from hospital to home.
Provide bedside/virtual clinical education to both the patient and caregiver to include hospital hook ups when indicated.
Building the relationships, being available to provide patient/caregiver/prescriber educational interaction related to patients transition from hospital/clinic/prescriber office to home.
Marketing specialty/traditional infusion capabilities to message every facet related to home infusion education/messaging on behalf of the UConn Health home infusion services.
Collaborate, Coordinate and Communicate with Nursing and Pharmacy intake to provide details needed related to a new patient referral and the patients seamless transition to home infusion services.
Aide the patient referral process
Performs other related duties as required.
FULL TIME EQUIVALENT SALARY MINIMUM: $89,378
