SUMMARY
The Clinical Care Coordinator (C3) proactively identifies and removes barriers that may impede department or system-wide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The ED C3 will act as the primary liaison between the Emergency Department and the Care Coordination Center (HUB), nursing, and support departments and in partnership with physicians to ensure accurate patient status and appropriate patient placement for effective, efficient patient-focused outcomes. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
QUALIFICATIONS
- Bachelor's Degree in a health relate field.
- Three (3) or more years clinical experience.
- Nurse Case Management, Emergency Department, Critical Care experience preferred.
Registered Nurse in the State of Georgia.
Basic Life Support (BLS) Certification.
Advance Cardiac Life Support (ACLS).
Certified Case Manager (CCM) or American Case Manager (ACM) Certification preffered.
