About the Team
The Projects for Assistance in Transition from Homelessness (PATH) program, supported by SAMHSA, focuses on engaging individuals in the community who suffer from serious mental illness and are experiencing homelessness. Our PATH team actively identifies, engages, and coordinates care for these individuals, addressing their healthcare needs and facilitating their transition to stable housing. As advocates and coordinators for this vulnerable population, this role involves extensive collaboration with community organizations, state benefits programs, and healthcare providers.
What You’ll Do
- Conduct assertive outreach to engage individuals in the community, addressing barriers to housing and improving social welfare and healthcare access/outcomes.
- Provide case management support, assisting clients in accessing community resources and actively building skills to enhance their quality of life.
- Collaborate with clients to meet project goals, including managing daily routines, applying for housing and employment/social welfare, accessing healthcare, and connecting with necessary treatment or community resources.

What We Need from You
- Clinical Associate I ($19.23 - $24.89): High school diploma or GED, one year of relevant experience working in health care, home health, or mental health service settings with an adult population who has experience a severe and persistent mental illness, and valid driver's license with a clean driving record spanning the past five years.

- Clinical Associate II ($21.00 - $27.18): High school diploma or GED (Associate degree preferred), two years of relevant experience working with mentally ill adults in relevant settings such alcohol, drug, developmental disability, physical rehabilitation, etc., and valid driver's license with a clean driving record spanning the past five years.
- Clinical Associate III: Bachelor’s degree in a human-services field such as social work, psychology, or similar disciplines. Alternatively, five years of relevant experience can be substituted for the degree, and valid driver's license with a clean driving record spanning the past five years.
Preference will be given to candidates with Peer Support Training and experience in Clinical Associate/Case Management/Peer Support roles, especially those who have completed the SOAR Adult Curriculum.
