Clinical Assistant ( Temporary 3/4 months)
Reports to: Director of Health Services
Classification: Full-time ( Immediate Hire)
Job summary:
The primary responsibility of the Clinical Assistant is to provide medical services under the supervision of Director of Health Services. The Clinical Assistant works closely with DOHS, QP, PC and DSPs in coordinating the medical needs of each resident.
Job functions:
- Schedules and monitors medical appointments for individuals receiving residential services.
- Obtains all necessary information prior to appointments, (i.e., Medical/Medicare cards, consult reports forms, lab forms).
- Transports residents to/from appointments and arranging assistance as needed.
- Obtains documentation from doctors following appointments and has a clear understanding of recommendations made/follow-up instructions prior to leaving appointment office.
- Expedites all physician consult sheets, medication orders or related paperwork and get them to the Lead Nurse in appropriate timeframe.
- Provides direct support/supervision to residents as needed in the home and community.
- Document in the client’s record (Therap) of appointments, labs, and other medical related information.
- Drop-off/picking-up of medical paperwork, such as FL-2s, LOCs, physician orders, etc. as needed.
- Serves as a member of the interdisciplinary team in planned meetings as needed.
- Assists with repairs or replacement of adaptive equipment for residents.
- Maintains confidentially of client’s information at all times.
Additional Responsibilities: Other responsibilities and duties may be assigned as needed. He/she will be provided with a company’s cell phone and a computer/laptop.
Schedule: M-F 8 am-4:30 pm (weekday), occasionally early am for earlier medical appointments
