Radiant Health Centers (RHC) seeks a Client Support Services Case Manager. This position conducts intakes, provides support and resources, completes psychosocial assessments, and assists monolingual Spanish-speaking prospective clients who are receiving medical care.

With a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options. With a focus on underserved communities, we seek candidates who are passionate in providing care to clients of all ages, genders, and backgrounds.

RHC is proud to be the only LGBTQ+ medical clinic in Orange County. We are actively working to become a Federally Qualified Health Centers (FQHC) in the near future.

Client Support Services will perform:

  • Enroll clients into services, assessments and service planning of medically stable clients.
  • Education of clients and their support systems regarding HIV disease, treatment and prevention, community resources and public benefits.
  • Assistance with applications for federal, state disability, general relief, Medi-Cal/CalOptima, etc.
  • Maintain regular follow up with clients on assigned caseload and assess for ongoing needs.
  • Maintain communication between inter and intra agency service providers to coordinate services.
  • Collaborate with agency departments to support client needs.

The ideal candidate will have the following qualities and qualifications:

  • Ability to work with diverse populations.
  • Demonstrate a positive caring attitude, be client focused; value and respect others.
  • Must exercise patience, understanding and concern for each client’s health, safety, development, and enjoyment of life.
  • Prefer a Bachelor’s degree or comparable case management experience.
  • Bilingual (English and Spanish) Required
