Summary
This Certified Registered Nurse Practitioner position within Transitional Care in the Community Living Center is located in Pittsburgh Pennsylvania at the Aspinwall HJ Heinz Facility. The CRNP is accountable for promoting patient safety, continuity/quality of care and general well-being of the patient. The CRNP will provide specialized care through traditional face-to-face mechanisms, as well as virtual electronic/tele-medicine mechanisms, to provide efficient, Veteran-centric care.

Duties
The HJH CLC is comprised of multiple neighborhoods/units. Some of these units are separate physical areas which provide specialized patient care and include: 1) Hospice Unit; 2) Memory Care Unit (MCU); 3) COVID Observation Neighborhood (CON); 4) COVID Recovery Unit (CRU); and 5) Road 2 Recovery (R2R) Neighborhood. The remainder of the units can accommodate veterans requiring other services and functional programs such as: 1) Subacute Rehabilitation; 2) Intensive Rehabilitation Program (IRP); 3) Skilled Nursing Care; 4) Long-Term Care (LTC); 5) Hemodialysis (HD); 6) Intensive Pain Recovery Program (IPRP); 7) Guardianship; and 8) State Veterans Home Transitional Care Program. The residents of the CLC are a mix of both short-stay (i.e., < 90 days) and long stay (i.e., >90 days) Veterans. The incumbent will be expected to complete admissions, clinical assessments, interpret diagnostic testing, determine appropriate patient workup and treatment plan. The incumbent will work closely with other Nurse Practitioner and Physician providers in a collaborative fashion to ensure the veterans' care is completed in a timely manner. The incumbent will be responsible for ordering appropriate specialty service consultations as well as coordinating appropriate collaborations with team members outside of the CLC and implement and assure continuity of individualized treatment plans. The incumbent is responsible to assure integrity, uniformity and quality of clinical nursing practice within the CLC as it relates to emergent, long-term care, and primary care needs. They will educate the veteran on disease process, what to expect from testing, potential treatments, and diagnosis. The incumbent will be expected to complete all aspects of clinical evaluation when appropriate including work-up, diagnosis, and treatment plans. The incumbent identifies and institutes preventive and treatment measures and monitors results of care delivered. The incumbent will be responsible for administrative duties required and to report progress and service line needs as necessary to clinical leadership. They will always act on the best interest of the veterans and the VA. They will train under CLC Nurse Practitioner and Physician providers to develop expertise in management of both emergent and chronic medical issues in line with the capabilities of the clinical setting. The Nurse Practitioner will work as a member of the health care team that will promote and maintain health by providing the following medical services:
Major Duties include but are not limited to the following:
1. Obtaining medical histories through chart review, telephone histories, face to face visits, and virtual visits.
2. Providing and coordinating off-tour medical care for GEC patients including establishing diagnoses, formulating, and implementing care plans, providing follow-up care and leading codes.
3. Evaluating diagnostic tests to determine the need for further health care or specialty care needs.
4. Ordering diagnostic studies and other special tests, such as Magnetic Resonance Imaging (MRI), Computerized Tomography (CT) scans, and ultrasound.
5. Carrying out health promotion, disease prevention, and patient education activities which include reviewing pertinent medical information with the patient/POA to include explanation of disease processes, diagnostic studies and treatment plans.
6. Ordering laboratory specimens.
7. Promote health of the patient and protect from disease. Provide health counseling as needed.
8. Assessing, treating, and determining disposition of patients with emergent conditions consistent with medical staff policy.
10. Ordering ancillary services, such as, pharmacy, social services, physical medicine and rehabilitation therapies, prosthetic services, etc.
11. Ordering VA specialty service consults and initiating requests for non-VA medical care and consults in accordance with VHA and local facility policy and procedures.
12. Writing orders for or prescribing medications.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE).
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Nurse Practitionersmust be licensed or otherwise recognized as a Nurse Practitioner in a State and maintain full and current certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.

Preferred Experience and Certifications - Certificate in gerontology and/or Family Nurse Practitioner Certification in-patient hospital experience, geriatric experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.
Physical Requirements:
  • Physical demands include repetitive body movements including the following: frequent reaching, standing, walking, and lifting of equipment and/or patients to provide patient care. This position requires extension of the hand(s) and arm(s) in any direction including above or below shoulder level, during the provision of patient care, including retrieval of needed equipment, supplies and to make contact with the patient. This position requires the maintenance of an upright position to perform duties associated with patient care. This position is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about accomplish tasks, such as assisting with patient ambulation and moving one patient area to the next. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next, i.e.- from the exam table to the chair, and in the movement of equipment or supplies.
  • Must meet Employee Health mandates as a condition of employment.

Additional information
Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
