Pathways Healthcare is a comprehensive, physician and nurse practitioner-led, medical rehabilitation home health organization. Pathways Medical Rehabilitation Program offers patients an alternative to an institutional short term rehabilitation. We’re looking for a compassionate, forward-thinking COTA who excels at assessing patients’ strengths and weaknesses and devising creative but effective treatment plans. Many of our patients are geriatric, have multiple chronic co-morbidities and need this program to age in place in the comfort of their own homes.
Position Details:

Occupational Therapist Assistant Responsibilities:
- Treat newly evaluated patients and follow creative and intensive plans of care
- Follow detailed treatment and care plans for each of the patients
- Communicate and coordinate with patients’ physicians to ensure a consistent level of care
- Treat patients in their homes and teach them how to safely ambulate in their own environment
- Communicate and document on patients’ progress
- Assist administrative staff with billing and reporting inquiries
- Comply with federal and state Certified Occupational Therapy Assistant regulations
- Work and connect with other therapy and nursing staff to promote excellent communication
- Ensure outstanding communication with patients, families, physicians and teammates
- Strive for the best patient experience and outcomes possible
Occupational Therapist Assistant Skills:
- 1+ years’ professional COTA experience preferred
- Valid state COTA license
- Exceptional work ethic and dedication to patient success
- Excellent verbal and written communication skills
- Basic computer skills
- CPR and first aid certification required
