Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+. We have and multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.
The National Association of Rehab Providers & Agencies (NARA) awarded Transitional Care Management their "Impact Award" for 2023 for our Life Skills Programming. This center is using that very same "Life Skills Program" and changing lives every day. Ask us about it during your interview - we'd be delighted to share our Social Services success stories. It's so much more than what your current Psych/Social team is doing now!
Job Description
What you will do in this role: In our facility we feel the CNA is pivotal to the satisfaction of a patient/resident. CNAs make a direct impact on a patient's recovery and stay. You will help patients reach their goals and work side by side with a highly skilled multidisciplinary team.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
Each center is unique; offering you a variety of long-term career opportunities.

- Salary: $18.00/hour - $24.50/hour based on experience + benefits
- Schedule: Full time
- Shift: PM
- 3:00 pm - 11:00 pm with an every other weekend rotation required

- The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.
- Many are able to age in place, and about 25% return to the community.
- We have several programs encouraging engagement, but the most popular spot in our center is the game room.
- We love new grads and work with students from multiple schools/universities in the area.
- The Metra and the Green Line are just a few blocks away

Qualifications
Certified Nursing Assistant (CNA): Sometimes called a Certified Nurse (or Nurse's) Assistant or Aide.
What we need from you:
- Certified Nursing Assistant (CNA) certification in IL
- Prior experience in a hospital or short-term rehab unit within a skilled nursing facility is ideal, but New Grads are welcome
- Electronic medical records experience (Point Click Care) is helpful
