CDPAP SCHEDULING COORDINATOR

We are looking to hire a Scheduling Coordinator (CDPAP) for our office location in Brooklyn, NY.

Bilingual Scheduling Coordinator Qualifications:

• Must speak Russian;
• Must have at least 1 year of experience IN HOME HEALTH CARE Coordination;
• Proficient in MS Office and computer competence a must
• Must have prior experience in HHA Exchange;
• Excellent customer service, attention to detail and communication skills a must
• Accountable, self-motivated and organized team player
• Ability to maintain confidentiality important

Bilingual Scheduling Coordinator Responsibilities:

• Be responsible for the assigned patient caseload
• Staff new cases accepted by the Agency on a daily basis
• Address patient requests and aide inquiries
• Resolve issues and concerns related to provision of patient services
• Monitor daily clock in and out time attendance during aides' work shifts
• Perform communication with contracted health plans regarding the assigned patients
• Perform on-call after business hours/ over the weekend and maintain complete on-call log
• Maintain accurate and complete patient record in the agency's internal system
• Submit daily and weekly progress reports to the supervisory staff
• Participate in handling other priority work-related projects as assigned by Administrator
