- Responsible for assisting the residents of Sacred Heart Adult Home with meeting their psychosocial, financial, spiritual and medical needs (i.e. social history, predictable and acute Wellness meetings).
- Will assist residents and families with orientation and adjustment to Sacred Heart Adult Home and orient them to the facility.
- Available to address any questions or concerns that the resident or family may have regarding their stay at Sacred Heart Adult Home. The Case Manager will communicate with the Administrator and Director of Health Services as well as all staff to meet the needs of the residents.
- Responsible for on-going communication with residents and family members while at Sacred Heart Adult Home to address their needs.
- Adheres to the established policy and procedures of the facility and ensures the NYS DOH requirements are met.
- Investigate and report to the Department and, as applicable, to the Justice Center, allegations of occurrences which would constitute a reportable incident.
- Complete initial and periodic evaluation, as directed by regulation, of the needs of a resident and of the capability of the facility program to meet those needs.
- Assist each resident to adjust to life in the facility.
- Assist each resident to maintain family and community ties and to develop new ones;
- Encourage resident participation in facility and community activities;
- Establish linkages with and arranging for services from public and private sources for income, health, mental health and social services.
- Assist residents in making applications for, and maintaining, income entitlements and public benefits (i.e. VA benefits, SSI, social security, long term care policies, etc.).
- Assist residents in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual's health and mental health needs.
- Maintains Residents Rights at all times.
- Participates in monthly Resident Council and Family Council, when appropriate.
- Remains up to date with DOH regulations and residents' rights.
- Assist residents in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident's health or mental health, including but not limited to:
- health and mental health services;
- dental services; and
- medications;
- Provide information and referral.
- Coordinate the work of other case management and service providers within the facility.
- Utilize and cooperate with external service providers on behalf of the residents.
- Assist residents in need of alternative living arrangements to make and execute sound discharge or transfer plans.
- Assist in the establishment and operation of a system to enable residents to participate in planning for change or improvement in facility operations and programs and to present grievances and recommendations.
- Provided each resident such case management services as are necessary to support the resident in maintaining independence of function and personal choice.
- Maintain a system of recordkeeping which documents the case management needs of each resident and records case management activities undertaken to meet those needs.
- Provide, without charge, space for residents to meet in privacy with service providers
- Allow access to individual residents who request services.
- Identify persons in need of services and assist external service providers in establishing a relationship with these residents.
- Work with external service providers in executing a plan for service for individual residents.
- Assist in arranging for transportation as necessary to ensure that residents are able to attend required services provided in an external location.
- Participates in orientation of employees to the needs of the residents.
- Performs any related and assigned duties as necessary.
- Attends all mandatory in-service education programs, including fire and evacuation drills.
- Follows the facility's policy and procedures for fire safety, disaster, and evacuation.

QUALIFICATIONS
- A Bachelor's or Master's Degree from an accredited college or university with major work in human resources or service delivery and one year of full time experience in the provision of services to a dependent adult population or
- An Associate's Degree from an accredited college or university with major work in human resources or service delivery and three year of full time experience in the provision of services to a dependent adult population.
- Able to effectively listen to residents and/or family members to understand the residents' needs and match them with the proper services.
- Must analyze problems and challenges of residents to direct them to the proper services which include evaluating the effectiveness of proposed treatments, solutions and other services.
- Must be able to endure constant interactions with residents and/or family members who likely have different perspectives or interests. Empathy will often be of need especially when progress occurs slower than anticipated or desired.
