Case Manager

Crossroads Programs, an affiliate of Apis Services, isseeking a Case Managerto join our Capable Adolescent Mother's (CAM) team of professionals in Cinnaminson NJ.
At CAM, you will provide services to girls, age 14 to 21-years-old, in the child welfare system with emotional and behavioral issues, admitted while pregnant and can stay until infant's 2nd birthday.

Qualifications:

• BSW or BA, BS, in Human Services, Psychology, Sociology or related fields of study
• At least three years of experience working with at risk youth.

Summary of Essential Duties:

• To advance ethical practice and professionalism while supporting protection emotional stability and healthy childhood development through service coordination, linkage, advocacy, etc.
• Compile all relevant information and documentation pertaining to youths
• Participate in service coordination on behalf of youth and families, under supervision of the MDT and/or licensed therapist
• Work in tandem with the licensed therapist, youth, family/guardian and all other members of the MDT providing care coordination input in the development of individual service plans, treatment plans and IMDS tools
• Serve as School, Community, and Ancillary liaison.
• Represent the youth and family on the agency's behalf as it relates to screening, placement, court appearances, etc.
• Remain in consistent contact with all members of the youths MDT (internal/in house and external)
• Provide youth-centered advocacy
• Provide referral and linkage to ancillary and or specialized treatment services and interventions
• Routinely conduct school, home, and ancillary visits
• Schedule and attend special appointments (medical, allied, etc.)
• Responsible for correspondence on behalf of the youth and family-treatment team meeting invitations, as well as monthly/quarterly plan dissemination to members of the MDT
• Responsible for youth file management
• Participate in treatment conferences, individual educational plan, child family team, emergency review meeting etc.
• Actively participants in, MDT, House Rounds, Clinical Rounds and Record Reviews
• Responsible for PerformCare authorization status and ensuring Medicaid coverage eligibility

Candidate must pass:

• Criminal Record History Clearance
• FBI and Child Abuse History Clearance
• Physical & Drug Screening

Candidate must possess:

• Valid NJ/PA Driver's License.
• Ability to attend and successfully complete all sponsored training
