Job description

Dane County Human Services Department (Children Youth and Families Division) has a current vacancy for full time (1.0 FTE) Case Manager in the Target Safety Support Funding unit. The office location for this position is 2306 South Park Street, Madison, WI. This position will have a hybrid schedule.
The Eligibility List created will be used to fill future vacancies over the next six (6) months.

DEFINITION

Under the general supervision of the CYF Supervisor the Case Manager work closely with children, adults, families, guardians, service providers and natural supports. Case Managers promote self-determination, community belonging, valued social roles, the sharing of ordinary places, opportunities for contribution, and choice while seeking to ensure health and safety.

EXAMPLES OF DUTIES

Work as part of a team to conduct ongoing assessment and analysis of child safety, parent protective capacities and needs in partnership with families to establish and prioritize goals and service needs. Establish ongoing relationships with children, youth, and parents to encourage and motivate change behaviors necessary to maintain personal and family safety. Establish supportive teams and facilitate meetings that include children, youth, parents, out of home caregivers and other key partners who provide services and support to the family. Conduct home visits to ensure all elements of an in-home safety plan or protective plan are followed. Teach parents hands on parenting techniques to increase parental protective capacities and ensure child safety. Develop and implement crisis plans around children and adults with persistent social and emotional dysregulation. Support parents in their recovery from substance abuse disorders. Be a social support for parents in times of crisis. Assist in carrying out all duties identified in an in-home plan. Complete required documentation based on information gathered through ongoing assessment in a timely manner. Participate in relevant in-service trainings programs to maintain proficiency in necessary skills. Perform other duties as assigned.

Education, Training and Experience:

Requires a Bachelor's Degree in a human services field (social work, psychology, sociology, nursing, counseling, etc.) and one (1) year of professional experience working with individuals from multi-ethnic communities as well as from various socio-economic backgrounds. This experience includes working with parents or children struggling with mental health dysregulations and parents in active stages of drug use and recovery. Preferred certification in teaching an evidenced based parenting curriculum.

Special Requirements:

Requires a valid driver's license, access to reliable personal transportation and a good driving record.

Background Check Statement: Some positions may require a criminal background check which can include fingerprinting due to the nature of the job's responsibilities. Wisconsin's Fair Employment Law, s. 111.31 - 111.395, Wis. Stats., prohibits discrimination because of an arrest or conviction record. Bilingual Requirements: For designated bilingual positions, candidates must pass a bilingual test (e.g., Spanish - English, Hmong - English, etc.)

Physical and Environmental Work Requirements:

Remote work and Office environment. Must be available to work occasionally on evenings or weekends. Ability to see and hear within normal ranges with or without adaptive devices. Ability to use finger, wrist, hand and arm motions. Ability to sit and stand.

Basic understanding of human development and behavior. Knowledge of current human service philosophies and approaches such as self- determination, person-centered planning, community inclusion, trauma informed care and asset based community development. Knowledge of, and experience applying case management methods, techniques, policies, procedures and treatment techniques. Knowledge of investigation procedures and applying motivational interviewing techniques. Experience facilitating group and individual counseling for different clientele. Knowledge of computer based applications including word, excel and other related software. Ability to use a computer to generate reports, complete forms, and document case activity. Ability to prepare and maintain complex records and reports. Ability to develop positive, long-term relationships with people, understand their concerns and act to address them in a timely, respectful and effective manner. Ability to quickly acquire and apply knowledge of laws, regulations, policies and procedures relating to the target population. Knowledge of and ability to access community resources. Ability to prioritize, calmly manage and resolve urgent or crisis situations. Capacity to organize large amounts of information and manage complex situations to achieve the most effective individual and programmatic outcomes. Ability to be decisive, consistent and diplomatic. Excellent personal, written and verbal communication skills. Ability to follow oral and written instructions, autonomously meet deadlines, participate in and appropriately use supervision. Ability to participate effectively as a team-member. Ability to engage and work with people from diverse ethnic and cultural backgrounds with understanding and respect for the individuals and their differences.
