Case Manager - Substance Use Disorder - Pontiac Mi

We have locations in Plymouth, Canton, Brighton, Dearborn Heights, & Madison Heights. All services will integrate Substance Use Disorder, mental health, and primary health care treatment in cohesive and client-centered programs.

POSITION SUMMARY:

We are seeking responsible and compassionate Case Managers to help locate applicable care and resources for our clients and to transport clients to/from approved resource providers. The ideal candidates will take initiative in instituting case management services and communicating with clients in effective ways. Experience with persons with Substance Use Disorder and/or co-occurring disorders is preferred.

REQUIREMENTS:

• Bachelor's degree in a related human services field (social work, psychology, criminal justice, etc.). CAADC, CADC or Development Plan from MCBAP.
• Driver's License and a clean driving record. Applicants must be comfortable driving clients in their personal vehicle.
• Knowledge of Microsoft Office (Word, Excel, PowerPoint)
• Be familiar and comfortable with the culture and characteristics of persons with Substance Use Disorders and mental health disorders. Provide a welcoming and accepting environment in which this population and their families can receive services.
• Be able to multi-task
• Be confidential and a good listener
• Have effective communication skills, both written and verbal
